25 | Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced case management inspection due to two incidents that were reported to the department. LPA met with Ola Bailey, Site Supervisor, who provided LPA with a tour of the facility. There were 14 children and 4 staff present upon arrival.
The purpose of the visit was to follow-up on the two incidents reported. The first incident was reported on 03/01/22 for physical plant plans that commenced on 03/21/22. The second incident was reported on 04/13/22 and it occurred on 04/12/22. Both incidents were reported via telephone within the required time to report.
LPA conducted interviews, obtained documentation, and took pictures during this visit.
Information reported to the Department on 03/01/22 indicated that the facility will have their parking lot repaved and construction would take place on 03/21/22 - 04/10/22. No staff or children were present at the facility during the construction dates. Information reported to the Department on 04/13/22 indicates that Parent #1 expressed concerns to staff.
Based upon information received from the interviews conducted it was determined that there is no immediate health, safety, or personal rights risk to children in care at this time.
There were no deficiencies cited during today’s inspection.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Ola Bailey, Site Supervisor, including, but not limited to Provider Rights, Appeal Procedures
