An unannounced Required – 1 Year Inspection was conducted on 09/07/2022 at 9:15 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 45. Licensing staff met with Maria Alvarez, Site Supervisor. The program is a preschool with a toddler component. LPA observed separate entrances for each component during visit. Both programs operate Monday thru Friday, Head Start from 8:15 AM to 2:30 PM and Early Head Start 8:00 AM to 3:00 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. Required postings were observed on bulletin board at entrance of facility and in each classroom. Per Site Supervisor, Facility just reopened on 08/22/22 and scheduled a disaster drill for next month. Early Learning Manager (ELM) Leslie Gamero, joined the visit at 10:20 AM.

At the initial start of the site visit, there is a total census of 23 children, 20 preschoolers and 3 toddlers, and a total census of four teachers present. The facility is made up of two classrooms, RM 1 (Head Start) and EHS (Early Head Start) RM 1. LPA observed Staff #1 and #2 in Head Start RM 1 with eight preschoolers and EHS RM 1 with Staff #3 and three toddlers.

Beginning at 9:37 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed each classroom to have learning stations with Science, Dramatic Play, Manipulative Toys, Art materials, and a quiet play area available for children in care. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per Site Supervisor, linens and blankets taken home weekly for washing. Availability of drinking water was reviewed. LPA observed water coolers in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed each classroom to have access to their own restrooms. LPA observed one toilet and one sink in Head Start Room 1 and two toilets and two sinks in Early Head Start Room 1. LPA observed a walk up diaper changing table in Early Head Start Room 1.

A first aid kit is kept in cabinets in each classroom. At 9:41 AM, LPA observed medication in EHS RM 1 stored for C1 meeting Title 22 Regulation requirements. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced in 07/08/22. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items stored in Kitchen area that is inaccessible to children in care.

At 9:45 AM, food preparation areas were toured for safety, cleanliness and proper equipment. Facility prepares food in Kitchen through EHS RM 1. LPA observed Kitchen is inaccessible to children by various doors with knobs inaccessible to children in care. Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Facility serves Breakfast, Lunch, and a PM snack.

At 9:47 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed 12 children with Staff #4 and #5 playing outside. LPA observed art materials, crawling space, child size tables and chairs, outdoor play equipment, tricycles, concrete bike path, and cushioning through out play area to absorb fall. Required shade and fencing were inspected. LPA Chicote advised Site Supervisor that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to water coolers during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed. Per ELM, Preschool and Toddlers share outdoor play yard. LPA did not observe a waiver posted and ELM states they do not have a waiver to share outdoor play yard. LPA observed staggered outdoor play schedule for preschools and toddlers at time of inspection.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed 23 of 23 children signed in with full signatures. Personal Rights of children were observed by LPA.

Children’s Records were reviewed. LPA observed 5 of 6 staff have current AB1207 training. CPR card expires on 08/04/2023 for Site Supervisor. LPA was unable to review required personnel documents due to staff files being in Administrative Office. LPA will review staff files at a later time.

Incidental Medical Services: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Facility Representatives:
Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 09/07/2022.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
