25 | Licensing program analyst (LPA) conducted an unannounced case management visit to the above mentioned facility on 02/27/24 at 3:05 PM. LPA was met by licensee, Shameka Nicholas who guided analyst on a tour of the facility. Also present during this inspection was S2 and S3. LPA observed 5 children in care during the visit. The home was observed to be clean and in good repair.
The purpose of this visit is to issue a citation for a deficiency involving personal rights of a child(ren) in care. Individuals interviewed stated that children in care were bitten by the licensee's pet dogs, Blue and Peaches. During the visit, LPA observed 4 dogs in the home. Licensee states she has never been told by the children or their parents that her pet dogs have bitten any of the children. LPA advised licensee that a type A citation under California Code of Regulation (CCR) section 102423(a)(2) will be issued.
LPA R. Derraco informed licensee Shameka Nicholas that this report dated 02/27/24 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.
Also, LPA R. Derraco informed the licensee Shameka Nicholas to provide a copy of this licensing report dated 02/27/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted, appeal rights provided and report was reviewed with the licensee Shameka Nicholas.
