25 | Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced Case Management Legal inspection to the above facility. LPA arrived at 4:20PM and met with Cynthia Zuniga, Director, who guided analyst on a tour of the facility. There were 8 children with 3 staff present upon arrival. LPA was later met by Victor Sanchez, Licensee.
The purpose of this inspection was to review the Decision and Order that was ordered on 10/20/21 for Destiny Pauline Rosales. The Decision and Order is based on the Accusation (Criminal Record Exemption Denied) for Destiny Pauline Rosales that was set forth on 09/10/21. LPA toured the facility to ensure that the individual named is no longer working and/or volunteering at the facility. The purpose of the inspection was discussed with the Licensee and Director and a copy of the Decision and Order was provided during the inspection.
LPA obtained a signed declaration during today’s visit. Per Licensee, Destiny Pauline Rosales has not been present or employed at the facility for several years.
Based on the evidence obtained during today’s visit, LPA Ramos has verified that the individual Destiny Pauline Rosales is not present, employed, or volunteering at the facility. LPA observed that Destiny Pauline Rosales is not associated to the facility and advised Licensee to submit an updated LIC 500 to list current staff.
There were no deficiencies cited during today’s visit.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Victor Sanchez, Licensee, and Cynthia Zuniga, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
