Licensing Program Analyst (LPA) Katrina Chicote conducted an unannounced complaint inspection on 10/07/2022 at 1:15 PM for the purpose of delivering findings to the above allegation. LPA singularly toured the facility. There were 17 napping children with three staff members (S2, S3, and S4) upon arrival to facility. All staff present had criminal record clearance.

During the course of this investigation, LPA interviewed Director, staff, parents, and children. All pertinent documentation was collected including Faciilty Roster and diaper changing logs. Interviews did not provide any corroborating information in regards to allegation. All staff interviewed provided consistent statements in regards to diaper changing policy and procedures. Parent interviews consistently state being satisfied with level of care being provided at facility and had no complaints similar to allegation. LPA observed staff changing diapers during inital 10-day inspection consistent with procedures provided in interviews. Children itnerviews made no disclosures in regards to allegation.

Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are UNSUBSTANTIATED.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 10/07/2022.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview was conducted and report was reviewed with the Director, Victor Sanchez.
