Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1-year inspection on 07/13/2021. LPA arrived at the facility at 10:50 AM. LPA met with designated teacher to act as a Director, Karine Khachatryan who guided analyst on tour of facility. During this inspection Diana Menedjian, Director arrived to the facility. This is a preschool program which consists of 10 classrooms; RM Crickets (4-5 year old), RM Dolphins (4-5 year old) , RM Monkey (4-5 years old), RM Duckling (4-5 year old), RM Frogs (3-4 year old), RM Pandas (3-4 year old), RM Buzy Bees (3-4 year old), RM Lady Bugs (2-3 year old), RM Butterflies (2-3 year old), and RM Bumble Bees (2-3 year old). Facility operation hours are Monday to Friday for 8:00 a.m. to 5:30 p.m.. Per teacher Knarik Khachatryan currently there are 110 children enrolled in the program.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: RM Butterflies: no kids during summer; RM Bumble Bees: 2 staff with 9 preschoolers, RM Lady Bugs: 1 staff with 8 preschoolers, RM Frogs: 1 staff with 11 Preschoolers, RM Buzy Bees: 1 staff with 9 preschoolers, RM Pandas: 1 staff with 11 preschoolers, RM Ducklings: 1 staff with 10 preschoolers, RM Dolphins: 1 staff with 11 preschoolers, RM Monkeys: 1 staff with 10 preschoolers, and RM Crickets: 1 staff with 10 preschoolers. The following was observed during tour of facility:

At approximately 11:10 AM LPA Eivazian began facility tour with teacher Knarik Khachatryan. All areas identified on the facility sketch were inspected. LPA observed 89 children with 12 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. All children were observed to be under visual supervision of a teacher at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. There is a sofa in the office. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast, lunch and late lunch. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available indoor, water pitchers and disposal cups are being used inside the classrooms and there are water fountains outside for outdoor play time. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has functioning carbon monoxide detectors that meet statutory requirements. The licensee takes measures to keep the facility free of flies, other insects and rodents. There are required 2A 10BC fire extinguisher available on the premises, that were serviced on 03/16/21.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this inspection. Areas around and/or under climbing equipment, swings and slides have cushioning material to absorb a fall; climbing apparatuses are single level only. Director states there are no weapons or firearms on the premises.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff Records were reviewed to ensure that a health screening report is on file. LPA reviewed five staff file and all 5 reviewed files were completed. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. LPA reviewed 12 children files and all 12 reviewed file were completed. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.

SB792 (Immunization Requirements for Staff and Employees) was discussed with the Licensee. LPA observed immunization records on five reviewed staff files.

Required mandated reporter training certificate was available in five reviewed staff files.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following was discussed:
· Reporting Requirements.
· Individuals who are 18 years of age or older living in the home must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately. Failure to obtain a criminal record background check clearances prior to initial presence in the home will result in an immediate $100.00 dollar or more per day Civil Penalty.
· The facility shall ensure that each child is accorded the following personal rights: to be accorded safe, healthful and comfortable accommodations.
· All personnel shall be in good health and shall be physically and mentally capable of performing assigned tasks; personnel that pose a threat to the health and safety of children shall be relieved of their duties.
· All personnel shall be given on the job training in housekeeping, sanitation principles and universal health precautions.
LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.
At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Director, Diana Mededjian, Appeal Rights and Procedures explained and provided at 2:00 p.m..
