An unannounced annual/random inspection was conducted today by Licensing Program Analysts (LPAs) Ariel Cazares and Thelma Razo who met with Site Director Jennifer Castillo. Education Coordinator Mervin Caiza-Rodriguez arrived shortly after. The facility operates on the property of Pasadena Covenant Church. The hours of operation are from 6:30 AM to 6:00 PM. There were a total of 33 children and 6 staff observed.

The facility consists of 3 classrooms: Fullday #1, #2, & #3. Currently room #3 is not in use. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings were observed in each classroom. Each classroom has drinking water in form of water jugs and disposable cups available to children. Children's restrooms were inspected. LPA Cazares observed in the restroom of classroom #2 a cabinet underneath the children's sink without a latch. Upon inspection LPA observed a spray bottle of sanitizer inside, which can be accessed by children. LPA Cazares observed underneath the adult sink attached to children's sinks in the hallway, a bottle of floor cleaner in a cabinet not latched. This is near the children's sinks and can be accessed by children. Photos were captured. LPA advised Mr. Caiza-Rodriguez to make items inaccessible. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed. First aid supplies were observed in each classroom. The carbon monoxide detector in classroom #2 was tested and found operational.

The outdoor playground was inspected. There is an approved waiver with the Department for a staggered schedule for no more than 28 children outdoors at a time. LPAs observed there to be 15 children outdoors on this date. Outdoor area and equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. No hazards were observed.

The facility offers a breakfast, lunch, and afternoon snack. Snack/lunch menus were observed posted throughout the facility and were reviewed. Food items are kept in the refrigerator located in classroom #3. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety and cleanliness. The kitchen is located in the building adjacent to the program.

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s personnel and administrative records. Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met. Sign in and out sheets were reviewed and found to be accurate.

Children's files that are kept on site were reviewed. The main child's file and staff files are located at the main office: Options Early Learning Center’s main office, located at: 885 S. An exit interview was conducted with Education Coordinator Mervin Caiza-Rodriguez. A copy of this report and appeal rights were provided. A "Notice of Site Visit" and copy of the report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty.
