An unannounced annual/random site inspection was conducted by Licensing Program Analyst (LPA), Ariel Cazares, who met with Teacher Armine Ghaloosian. Education Coordinator Diana Castellanos arrived later during inspection. The facility is located inside the Villa Parke Community Center. This facility operates an am and pm session in each classroom: Classroom 1 operates 7:45-11:15 and 11:45-4:45; Classroom 2 operates 7:45-12:45 and 1:15-4:45.

LPA toured and inspected the classrooms. Classroom 1 had 12 children and 2 staff. Classroom 2 had 15 children and 2 staff. Classroom furniture and equipment were inspected for age appropriateness and its condition. Telephone service, heating, lighting and ventilation were evaluated. Storage space/area for children's belongings was inspected. The condition and age appropriateness of children’s sinks and toilets were inspected. LPA evaluated general sanitation and cleanliness of facility. Availability of indoor drinking water was observed in form of water jugs and disposable cups. The carbon monoxide detector for the building is located in the hallway and is hardwired to the smoke alarm system. LPA was unable to test due to the panel only being accessed by the fire department. LPA observed a red blinking light on the detector. LPA advised that a detector for the facility be installed so that it can be tested during inspections.

The outdoor playground is located directly outside of the classrooms. Outdoor area and equipment was inspected for safety, cushioning material, condition and age appropriateness. LPA observed small patches of cushioning material missing. LPA advised Ms. Castellanos to ensure the material is maintained to prevent any tripping hazards. At this time the wear and tear does not pose a risk to children's safety, but further wear and tear can create a tripping hazard. Required shade and fencing were inspected. LPA inspected the sandbox and observed it to be clean. LPA observed no bodies of water. Outdoor drinking water is provided in form of water jugs and disposable cups brought out from the classroom.

The facility provides breakfast and lunch to the am session and lunch and snack to the pm session. Meals and snacks are catered in and prepared and distributed to children on site. LPA observed the menus posted in the classrooms. Food preparation area was inspected.

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s personnel and administrative records. An exit interview was conducted with Education Coordinator Diana Castellanos. A copy of this report and appeal procedures were explained and provided.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
