25 | ANNUAL/RANDOM INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analysts (LPA) Anomeh Eivazian and Roxana Lopez conducted an unannounced random inspection. LPAs met with licensee, Anie Mardirosian who guided analyst on a tour of the facility. There were 3 children present during this inspection, two being infants. Ms. Mardirosian states that there are currently 6 children enrolled, children's roster was reviewed and is not current. Disaster drill log was available, last drill was conducted on 12/06/19. Facility operation hours are Monday to Friday from 7:30 a.m. to 5:30 p.m..
This is a one story home which consists of 3 bedrooms, 3 bathrooms, kitchen, dining room, living room (FIREPLACE: inaccessible), spare living room (child care room), converted garage (guest house), front yard and backyard (fenced). The children use the 1 child care room (spare living room), 1 rest-room in the child care room, and fenced back yard. Per licensee, parents drop off and pick up the children from back door which leads to the day care room. LPAs observed a safety gate was installed in the day care room which makes the rest of the home inaccessible to the children. Per licensee, areas off limits to children and parents include: 3 bedrooms, 1 & 1/2 bathrooms, living room, dining room, kitchen, front yard and converted garage (guest house). Family members residing in the home are 4 adults and 0 children. Licensee and her parents live in the main home who have clearances on file, and licensee's cousin, Loosineh Gharahbegloo lives in the converted garage (guest house) who has criminal record clearance on file.
All areas used by children were inspected for safety, comfort, cleanliness, telephone, ventilation and heating (central). The licensee states that there are no poisons in the home. The licensee understands that any poisons must be locked with a key or combination lock. Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children are inaccessible in all areas in the home.REPORT CONTINUES ON NEXT PAGE 1 of 3 |
