An unannounced annual site inspection was conducted by Licensing Program Analyst (LPA), Bardo Baluyot, who met with Lead Teacher, Amber Dichosa. LPA conducted a COVID risk self-assessment upon entry. Site Coordinator, Carmen Estrada arrived a short time later. There were a total of 11 children and 4 staff. Facility was in compliance with ratio.

Program currently operates a Full Day program in Portable A from 7:30 AM to 4:30 PM. Per Site Coordinator, Head Start program in Portables B & C will resume on 8/8/22 and operates a morning session from 8:15 AM to 11:45 AM and an afternoon session from 12:15 PM to 3:45 PM. Two separate classes operate within each portable. Currently, Portable A operates only 1 class due to low enrollment.

Classrooms identified on facility sketch were inspected. Furniture and equipment were inspected for age appropriateness and its condition. Telephone service, heating, lighting and ventilation were evaluated. Storage space/area for children's belongings were inspected. The condition and age appropriateness of children’s sinks and toilets were inspected. LPA evaluated general sanitation and cleanliness of facility. Availability of indoor drinking water was observed in form of water jugs and disposable cups.

Outdoor area and equipment was inspected for safety, cushioning material, condition and age appropriateness. Required shade, drinking water (water jug and cups are taken outdoors) and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. No bodies of water observed.

Kitchen was inspected for the following: Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness.

LPA inspected First Aid Kit’s supplies and completeness.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. After a complete inspection of the facility, there were no deficiencies observed on this date
according to California Code of Regulations Title 22 Division 12.

No children or staff files were reviewed during this visit as they are kept in the main office: Options State Preschool's main office, located at: 18455 E. Railroad St. City of Industry, CA 91748. Tel # (626) 854-3449

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the Site Coordinator, Carmen Estrada.
