On 02/26/2021, Licensing Program Analyst (LPA), Ariel Cazares conducted a virtual inspection via FaceTime with the purpose of this inspection is to provide technical assistance and ensure the facility is safe for the care and supervision of children in care. Due to COVID-19 state of emergency a Licensed Child Care Center wavier was granted on 02/05/2021 for the facility located at 1212 N. Pacific Ave, Glendale, CA 91202 to care for up to 10 school-age children K12th grade due to COVID-19. Hours of operation will be Monday-Friday 8:00am-3:00pm.

A fire clearance has been granted for a capacity increase of up to 86 in order to accommodate the 10 school-age children. The fire clearance dated 11//24/2021 also allows the use of two additional rooms, that were not initially licensed, to be used.

Check in will take place at the entrance of the facility outdoors. There is a handwritten system for sign in and out. A health check and temperature checks on staff and children will be conducted each day upon arrival. A parent board is available with COVID postings which was being updated due to postings becoming damaged. LPA advised to keep postings that address mask wearing, social distancing, symptoms to look for, and visitor policies. If children become ill during the day, they will rest in the isolation area located in the office. There is a staff restroom that can be used as the isolation restroom. Children’s authorized representative will be contacted immediately for pickup. Department of Public Health will be contacted in regard to any COVID-19 illness.

LPA was guided through the classrooms that will be used by school-age children. There were two school-age children present. There will be two rooms available to accommodate the 10 children. Children will stay with the same group as assigned and will not be moved from one group to the other. LPA observed age appropriate furniture and long tables that will used to provide social distancing. Children will keep their personal belongings with them at their table. Drinking water will be available in form of water jugs and disposable cups. There are two restrooms located in the same building. One restroom has 6 stalls and 3 sinks. The second restroom has 3 urinals, 3 stalls, and 3 sinks. There is a designated outdoor area for the school-age children. A tent and rest area such as a bench will be set up. Children will have their own separate play equipment.

LPA was toured through the preschool facility. There were a total of 6 groups in separate classrooms. Classrooms were observed to be separated by a partition that is from floor to ceiling. The following was observed: Monkeys 1- 7 children, 1 staff; Monkeys 2- 11 children, 1 staff; Lions 1- 9 children, 1 staff; Lions 2- 8 children, 1 staff; Elephants 1- 8 children, 1 staff; Elephants 2- 8 children, 1 staff.

The facility will maintain children files that include Personal Rights, emergency authorization cards and staff files that include proof of criminal background clearance. A copy of the signed waiver by Parent/Guardian of child will also be on file.

To further ensure the health and safety of the children in care, Community Care Licensing will provide on-going Technical Assistance (TA) to the facility. A copy of this report will be issued via email with a read receipt as confirmation of receipt. LPA requested report to be signed and returned to the Regional Office.
