Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection and required inspection to the above facility on 09/23/2021. LPA arrived at the facility at 8:50 AM and met with Nancy Darwich, administrator who guided analyst on tour of facility. During this inspection at 9:45 AM Lena Haiek, director/licensee arrived to the facility. This is a preschool program which consists of 3 classrooms; RM Elephants (4-5 years old), RM Lions (3-4 years old), and RM Monkeys (2-3 years old). This facility is located on Temple Sinai property. Facility operation hours are from Monday to Friday from 8:00 AM to 6:00 PM. Per the Director there are 76 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: RM Elephants: 21 preschoolers with 3 staff, RM Lions: 19 preschoolers with 3 staff, and RM Monkeys : 24 preschoolers with 4 staff. The following was observed during tour of facility:



At approximately 9:20 AM LPA Anomeh Eivazian began facility tour with Nancy Darwich, administrator. All areas identified on the facility sketch were inspected. LPA observed 64 preschoolers with 10 staff. Teacher- child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Per director, cleaning crew come daily to clean the facility. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean.

All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. There are chairs in the office. There is a napping cot available for an ill child. The staff restroom will used by ill children. This area is inaccessible to children. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast, lunch, and PM snack. Preschoolers eat either inside the classrooms or in the back shaded outdoor lunch area. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in all indoor classrooms and outdoors. LPA observed water dispensers and disposable cups for indoor and water pitchers and disposable cups for outside.

All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has functioning carbon monoxide and smoke detectors that meet statutory requirements in all classrooms. The licensee takes measures to keep the facility free of flies, other insects and rodents.

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Parents take linens home on Fridays and returned on Mondays.



There are two outdoor playgrounds on the premises. Outdoor playgrounds equipment are in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There is a sand box on the premises. Per licensee, sand box is covered every day after day care hours and staff check the sand box in the morning before children go out. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this inspection. Director states there are no weapons or firearms on the premises.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Medication: Incidental Medical Services (IMS) policy was discussed. There are no children enrolled that require IMS at this time. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. SAFE SLEEP: Facility is a preschool program and offer services to children ages 2 until entry in to first grad. LPA discussed the safe sleep regulations with director, Lena Haiek and discussed the Child Care Licensing Safe Sleep webpage at http://www.cdss.ca.gov/inforescources/child-care-licensing/public-information-and-resoucrces/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at http://www/cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. During this inspection total of 64 children were present in the facility and all 64 children were signed in. All present Staff Records were reviewed to ensure that a health screening report is on file. Ten Present children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Last Disaster Drill was conducted on 08/12/2021 as read by LPA from disaster drill log.

The following was discussed:
- Reporting Requirements.
- The facility shall ensure that each child is accorded the following personal rights: to be accorded safe, healthful and comfortable accommodations.
- All personnel shall be in good health and shall be physically and mentally capable of performing assigned tasks; personnel that pose a threat to the health and safety of children shall be relieved of their duties.
- All personnel shall be given on the job training in housekeeping, sanitation principles and universal health precautions.

LPA advised the licensee to access forms, regulations, providers information notices (PIN), and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA inquired into the licensee’s practices related to COVID-19. Licensee stated that they practice hygiene such as hand washing with the children. Licensee has hand sanitizer available in the drop off area and inside the classrooms. Licensee states they practice social distancing with children, take children temperature daily, licensee, staff, and children wear masks all the times indoor. Parents do not enter the classroom. In each classroom there is air purifiers. Teacher keep the classroom doors open as much as possible to get fresh air in each classroom. Per director, weekly they offer free PCR Covid-19 testing to all staff and volunteer parents.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
