25 | Licensing Program Analyst (LPA), T. Tran conducted a Case Management inspection at the above facility to follow up on the self-reported incident that occurred on 12/19/2019. The Monterey Park South West Child Care Regional Office received the incident report on 12/19/2019.
LPA conducted files review and obtained children's document. Based on the information that were available there was a substitute teacher and a teacher assistant with 17 children on the day of the incident. During clean up time, staff observed C1 and C2 were having a conflict putting toys away. While staff was encouraging both children to use their words and observed C2 pushed C1 against the shelves. C1 sustained two minor scrape and bruised the lower back. No medical attention required. Parents were not contact due to closer to dismissal time. At this time based on the available information it does not appear this incident was the result of a Title 22 violation for lack of care and supervision.

An exit interview was conducted; the notice of site visit must be posted for 30 days upon receipt. |
