25 | Licensing Program Analyst (LPA) Judy Mora conducted an unannounced Case Management- Incident inspection to address two separate incidents which occurred on 02/28/22 (Incident #1) and 03/02/22 (Incident #2). LPA Mora met with Lead Teacher, Eloisa Lopez Gonzalez. Upon arrival, LPA observed Lead Teacher outdoors with 02 children, 10 children with 01 staff in Room 1 and 11 children with 02 staff in Room 2.
The alleged incident that occurred on 02/28/22 was reported to the Department on 03/01/22 via telephone. The facility reported the incident as soon as they became aware.
The alleged incident that occurred on 03/01/22 was reported to the Department on 03/02/22 via telephone. The facility reported the incident as soon as they became aware.
LPA conducted interviews with staff and obtained documentation during this visit.
Incident #1 and Incident #2 were allegations of children's personal rights being violated. Based on all information obtained on this date, and interviews conducted with staff, there were no disclosures made that any child's personal rights were violated. Follow up will be conducted with the Administration Team in regards to documentation.
There were no deficiencies cited on this date.
The Notice of Site Visit (LIC 9213) – was provided and must remain posted for 30 days.
