25 | Licensing Program Analyst (LPA), Dayna Chambers conducted a Case Management Incident inspection to follow up on self-reported incident that occurred at 11/01/2019. The Monterey Park South West Child Care Office received the incident report on 01/07/2020 by Program Manager. Report stated that on 01/07/2020, a child (4 years old) told his mom that the teacher hits him and pointed to different areas of body. LPA met with Site Supervisor, Loretta Iberosi. LPA Chambers observed the facility and conducted an annual inspection during this time. LPA observed that the facility was properly cared for during the incident. On 02/13/2020, Site Supervisor and LPA Chambers toured the facility, at the time of the inspection all ratios were compliant according to Title 22 Regulations.
Based on today’s inspection, and interview conducted, the facility followed the appropriate Reporting Requirements, Notified Parents, and followed the center policies and regulations, This UIR was reported by the Program Manager and he conducted interviews with all of the teachers. There were no witnesses and this facility is one room open classroom,all parts of the classroom are visible and you can visually see all children and teachers. The teacher listed in the UIR is not currently working at this center since the date of the incident occurred. On 01/06/2020, the child's parent called the facility and is dis-enrolled the child. The center gave the parent 14 days to appeal.
At this time based on the available information it does not appear this incident was the result of a Title 22 violation and the facility followed the appropriate regulations to care for the children.

Notice of Site Visit shall be posted for thirty (30) days. Failure to maintain posting as required will result in a $100 civil penalty. An exit interview was conducted, and a copy of this report was provided to Site Supervisor, Loretta Iberosi. |
