On Friday, February 3, 2023 at 9:15 a.m. Licensing Program Analyst (LPA) Mayra Rivera conducted an unannounced annual inspection and met with director Lorretta Iberosi who guided LPA Rivera on a tour of the facility.

This is a preschool program that consists of one preschool classroom (open concept) and outdoor play yard. This program serves children ages 3 year to 5 years. This program operates Monday - Friday 7:30 a.m. - 5:30 p.m. Sign in and out takes place at the main entrance area. All areas identified on the facility sketch were toured and inspected both indoors and outdoors.



At approximately 9:20 a.m. LPA Rivera entered the preschool classroom. During this visit, LPA Rivera observed 18 preschool children playing with staff #1, #2, #3, and #4 present. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children’s personal items. LPA observed cleaning solutions stored on top of the file cabinets making it inaccessible for children to reach and inside the kitchen storage (off limits area). LPA advised to place the cleaning solutions inside a cabinet and place a child proof lock. LPA Rivera observed step trash bins with a tight-fitting lid.

At approximately 9:29 a.m LPA Rivera observed the restrooms to be in good condition with working toilets, sinks, hand soap dispenser and paper towels. LPA observed a total of 4 toilets and 4 sinks. Ill/isolation area children utilize the staff restroom located by the staff lounge.

At approximately 9:34 a.m. LPA Rivera entered the outdoor playground. LPA observed the playground surrounded by an apartment complex. LPA informed director Lorretta and staff #1 the importance of conducting daily safety checks to ensure the playground is safe for the children to utilize due to being surrounded by apartments

LPA observed the side gates to have double barriers such as a wooden gate and a push door gate locked to ensure children don’t have access to enter the apartment complex. LPA observed the play equipment to be in safe condition, free of sharp, no loose or pointed parts. LPA observed the surface of the outdoor activity space rubber mat to have big gaps which is a tripping hazard. LPA observed areas around or under climbing equipment, to be cushioned with granulated rubber padding that absorbs a fall. For outdoor drinking water, LPA Rivera observed a water pitcher and disposable cups. For adequate shade, LPA observed an eazy up zipped tied to the fence.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and the vents located high up on the walls. LPA observed the current room temperature to be at 73 degrees. LPA observed 4 fire extinguishers located inside the classroom and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 11/15/22. LPA Rivera observed smoke and carbon detectors and fire alarm system in the classroom. LPA tested the smoke and carbon detectors and are operable. LPA observed the first aid kit to be fully stocked with band aids, gauzes, adhesive bandages and antiseptics and located in the classroom and director’s office. LPA observed the fire/earthquake log and last drill conducted on 1/26/23.

LPA observed napping equipment cots to be stored in the staff lounge. Sheets and blankets are provided by the parents and the facility provides incase parents forget. For ill/isolation area children utilize the director's office and a mat is be available for children to use to rest if needed. During this visit, facility did not have any children with or on medication and no food allergies. Water lead testing conducted on 10/20/22.

This program provides breakfast, lunch and p.m. snack. The meals and snacks are prepared at an offsite kitchen and brought to the facility. A monthly menu is available and posted in the main entrance. For water drinking LPA observed a water dispenser and disposable cups. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents. LPA observed the refrigerator temperature to be at 40 Fahrenheit.

At approximately 9:50 a.m LPA observed on the bulletin wall the license, Parents Right's, Personal Rights, LIC 610 Emergency Disaster Plan, Facility Sketch, LIC 9148 Earthquake Checklist, PUB 269 Child Passenger System Poster, Drill log, monthly Menu and Daily Schedule. LPA also reviewed the children's roaster, children and staff files and the sign in/out sheets. LPA observed 18 parent signatures and 18 children present during the visit.

Director Lorretta Iberosi has incorrect pediatric First Aid/CPR certification, missing preventative health and safety (nutrition/lead) training. LPA observed child abused mandated certification (AB 1207) dated 10/10/22 and immunization against pertussis, and measles, and influenza declination. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com
Items discussed during this visit:

1. The following items are an immediate citation by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.


3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4.LIC 311A - Records to be Maintained at the Facility: LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6.Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7.Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles
9.Children’s Records: must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

10.Incidental Medical Services (IMS): This facility provides Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. (An IMS Plan was provided with the application and will be reviewed).

Director Lorretta Iberosi was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Lorretta Iberosi and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Lorretta Iberosi of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.



During this visit, two technical violations were given due to director having the incorrect Pediatric First Aid/ CPR certification, missing Health and Safety (nutrition/lead) certification and the surface of the outdoor activity space rubber mat big gaps which is a tripping hazard. Director has been informed due date for corrections 3/3/23.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with director Lorretta Iberosi.
