LPA met with designated facility Director, Elena Medina, who guided LPA on a tour of the facility. This is a preschool program licensed for 36 children. The facility conducts 04 different sessions in two separate classrooms. The sessions are Monday – Friday from 7AM-12 PM, 12:30-5:30 PM and 8AM-11:30AM, 12:30 PM - 4 PM. Per Director there are currently 67 children enrolled. All areas identified on the facility sketch were inspected. Upon arrival LPA observed 09 children with 02 staff in Room 1, 05 children in the restroom with 02 staff and 14 children with 02 staff in Room 2 in transition to the outdoor playground. Teacher-child ratios were observed and staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. The Licensee is within the conditions, limitations, and capacity specified on the license. Furniture and equipment was inspected to ensure that they are in good condition, free of sharp, loose or pointed parts. LPA observed that all indoor classrooms are clean and safe. All toilets and hand washing facilities are in safe and sanitary operating conditions. At this time, the Director's office is used as an isolation area. Parents are contacted immediately when children are determined to be ill. All materials and surfaces accessible to children are toxic free. Snack menus were reviewed. The facility provides AM snack, lunch and PM snack. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is brought in from the International Institute of Los Angeles (IILA). There is uncontaminated drinking water available in all indoor classrooms and a water jug and disposable cups are taken outdoors. All storage containers used for solid waste, including moveable bins have tight fitting lid at this time and are in good repair. *REPORT CONTINUES ON NEXT PAGE

Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are inaccessible. Storage areas for poisons are locked. Outdoor play equipment was observed to be in safe condition, free of sharp loose or pointed parts. The outdoor activity space was observed to be in a safe condition, and is free of hazards. Areas around and/or under climbing equipment and slides have rubber cushioning material to absorb a fall. The licensee states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Per Director there are no weapons or firearms on the premises. There is a carbon monoxide detector on site that meets statutory requirements.

Staff files were not reviewed due to being kept at the main St. Anne's ofiice. There is an Administrative file, license #198011690. Children’s Records were reviewed to ensure that each child has an Emergency and Identification form on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. The Director has valid Pediatric First Aid & CPR which expires 08/2020.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.

The licensee’s email address was obtained during this visit. The licensee was advised that email is public information.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing reprehensive. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Director, Elena Medina. The Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms. Director's name will be updated upon return to the Monterey Park Regional Office.
