13 | Licensing Program Analyst (LPA) Jacqueline Martinez conducted an unannounced complaint inspection to the family child care home above. LPA met with Licensee, Harriet Smith to follow up on a complaint inspection that was initially conducted on 5/24/19. A complaint was received on 5/15/19 that alleges that Licensee failed to address daycare child's diapering needs and that Licensee is using an inappropriate place for time out. During this inspection there were nine children present and two Staff members including the Licensee.
During the course of the investigation interviews were conducted with Licensee, Staff #2 and five children. LPA obtained copies of the children’s roster and reviewed the childrens files. Based on the collected information, LPA has deemed that there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore at this time the above allegations are unsubstantiated.
An Exit interview was conducted with the Harriet Smith, Licensee. Notice of Site Visit was provided.
Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
