Licensing Program Analyst (LPA) Cynthia Reyes, conducted an unannounced annual site inspection, increase in capacity and review of Incident report submitted, to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Cathy Gaeta, Site Supervisor and a copy of the Entrance Checklist for Child Care Centers form (LIC 125) was provided. A risk assessment was conducted prior to entering the facility. Due to COVID- 19 precautionary measures were taken during the entire inspection and LPA wore appropriate personal protective equipment. This is a Head Start preschool program which is operated by Plaza de la Raza.

This facility is licensed to care for 80 preschoolers, and is requesting to increase to 100. Full day program is in class rooms 6, 8, 10 and the requested added room 12, Monday-Friday 8 AM-3 PM. The 6 hour program is in class room 4 Monday-Friday 8 AM-2 PM. LPA observed the classrooms to each have their own entrance.

9:30 AM LPA was guided on a tour of the facility by Araceli Brion, Education Coordinator, who arrived at the facility to continue the inspection with the LPA and began the tour in the front lobby area. Physical check which include temperature being taken of children will be conducted by staff when the child enters the class room and documented. Use of hand sanitizer will also be available. Facility was observed to be adhering to local public health guidelines. (According to CDC guidelines, children under the age of (2) are not required to wear masks.)

The site supervisors office will be used as an ill/isolation area for children and the staff rest room right next to it will be used for the ill child. LPA observed an ill child cot/mat available. PAGE 1 OF 6

The tour continued with the preschool class rooms. Each class room has a Sign-In and Sign-Out sheet which was observed and signatures and dates and counts of children were inspected. Teacher child ratios were discussed in accordance with Title 22 Regulations. Staff names were recorded. LPA discussed that children should be under supervision and observation, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible to children. Araceli Brion states there are no poisons, weapons or firearms on the premises.

11 AM Furniture, equipment and toys were observed and inspected to be clean, safe and age appropriate and in good repair. Telephone service, heating, lighting and ventilation were evaluated. Preschool children have their own cubby to store their personal belongings. LPA observed napping equipment/bedding for children if needed as there is one half day program, and was inspected to be in good condition, appropriate storage and cleanliness. Linens are washed weekly by parents.

Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Menus for the past 30 days are available upon request. The facility offers breakfast, lunch and snack. Food is prepared by the central kitchen. Snacks were reviewed for availability, quantity and appropriateness to children in care. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. The licensee takes measures to keep the facility free of flies, other insects and rodents. All food is protected from contamination. LPA inspected and consulted that any contaminated food is discarded immediately. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

There is drinking water available indoors and outdoors by use of pitcher/jugs and disposable cups. Sippy cups & water bottles will be labeled if brought by the child. Page 2 of 6

The outdoor playground was inspected and the play equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in half of the outdoor play yard. LPA did not observe any bodies of water on the premises.

Age appropriate sinks, urinals and toilets were inspected for availability, good repair, water temperature, toilet paper, safety and sanitation. 2 bathrooms are available for preschool children, 1 with 4 sinks & 5 toilets, and the other with 2 urinals, 3 toilets & 4 sinks.

Araceli Brion was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Children’s Records were reviewed for completeness; Inspection of required forms were made and documented on the LIC 857. LPA will review Staff records, which are documented on the LIC 859. Staff present have proof against TB, measles, pertussis, and influenza, as well as current Mandated reporter.

11:50 AM LPA observed during staff file review, that there is at least one person trained in Pediatric CPR and Pediatric First Aid present during this inspection and at all times.

Medication: This facility provides Incidental Medical Services – IMS. 5 children medication policy was reviewed including storage of medication and equipment/supplies, and review of children’s, personnel, and administrative records. Children's roster was reviewed for completeness. Disaster drill log was available and last drill was document 09/27/2022 and is posted visibly for all to see in each class room.

First Aid supplies were observed in each classroom and were observed to contain the required first-aid supplies. According to Araceli Brion, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. No transportation is being conducted by the facility.

Update: Effects of Lead Exposure and the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD. were discussed.

Carbon monoxide detectors and smoke detectors are present and are in operable condition in the facility. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. LPA observed they are both located in each classroom, as well as the fire extinguisher type 2A-10BC and the valve on each indicated that they are all fully charged and each has a tag with a service date. of 08/11/2022. Fire extinguishers need to be serviced annually or as often as necessary.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. Increase in capacity was conducted and class room 12 was measured and inspected with the rest of the preschool program. Measurements in room 12 will be approved for the requested 20 additional preschool children. Total Outdoor measurements for the two preschool yards is capacity of 176. Total toilet/urinal and sinks can accommodate the request total of 100 preschool children.

***LPA highly recommends that a shade area be placed in the smaller preschool yard area, due to the play yard has been split in two by separation of a gate***

Incident report. LPA is also following up on an incident that was reported by the site supervisor Cathy Gaeta on 10/05/2022 regarding 2 confirmed children cases of hand, foot, mouth on 10/04/2022.LPA interviewed, reviewed and received records. LPA went over Incident report with Cathy and Araceli. Incident is regarding Hand, Foot, and Mouth outbreak in this infant/toddler program and in the preschool program. The Health department was notified as well as parents and were given all the required information.

Site supervisor states one child has returned to school without symptoms and a doctors note and 1 has not returned to school as of yet. Exposure Notice was sent home to the parents in the exposure class room, however LPA informed site supervisor to send exposure notice out to all parents in both programs infant and preschool. Site supervisor stated the entire school has a janitor that cleans daily and all staff has also cleaned and disinfected each of their class rooms. PAGE 5 OF 6

Based on information obtained on this date, no follow up is necessary regarding the incident report listed above. The facility followed all proper procedures per the parent hand book/admission agreement, incident reports were sent in properly and timely, all medical needs were met if needed, public health and parents notified.

There were no deficiencies cited during today’s inspection.

A notice of site visit was given and must remain posted for 30 days.
