Licensing Program Analyst (LPA) Mireya Garcia conducted an unannounced Case Management inspection due to an epidemic outbreak reported to the Department. Due to COVID- 19 precautionary measures were taken, licensing staff present during inspection wore appropriate personal protective equipment.

LPA met with Guadalupe “Lupe” Mennillo, Human Resources and Shau Ing Lay “Shirley Chang”, Acting Director/Teacher who guided LPA on a tour of the facility. Also present during this inspection was Paula Deng, RN (Public Health Nurse), Homer Boyd, M.D. (Physician Specialist LADPH), and Veronica Laguado, Environmental Health (DPH). Census was taken.

During the tour of the facility, LPA Garcia observed postings outside classrooms of letter dated March 29, 2023 for Gastrointestinal illness notification letter for students of South Hills Academy from County of Los Angeles Public Health, Homer Boyd, MD with written steps to prevent person-person transmissions. The facility was notified to increase the frequency of environmental cleaning in areas including bathrooms, kitchens, faucets, door handles, walls, and outside equipment. The facility was also notified to use disinfectants or bleach solutions on surfaces contaminated with Noravirus should be approved by the Environmental Protection Agency (EPA). In addition, the facility was advised to carefully and frequently wash hands.

During the closure facility will provide staff with all staff in-service training on; Proper handwashing, cleaning, disinfecting protocols & procedures. Based on information obtained during this investigation, the facility failed to implement the written directives provided by the County of Los Angeles Department of Public Health to decrease the spread of gastrointestinal illnesses. During today’s inspection, LPA observed children from classroom #128, #129 & #130 co-mingling in the outdoor play yard. Staff #2 disclosed classrooms #128, #129 & #130 are combined for outdoor play in the AM and PM and for departures. Staff #2 disclosed classrooms #126 and #127 classrooms are also combined during outdoor play in the AM and PM and for departures.

Staff #1 disclosed not increasing the frequency of cleaning of the outside equipment. Staff #3 disclosed using bleach and water solution to clean classroom, bathrooms and sinks. When LPA Garcia asked to see the storage of bleach solution Staff #3 stated the facility ran out of bleach today, 04/06/23.

At 9:39 a.m., LPA Garcia observed Staff #2 assisting a child with toileting and handwashing. LPA did not observe proper handwashing procedure implemented. Child was observed washing hands for less than 5 seconds and then child was transitioned back into the outside play.

During the inspection of the kitchen at 11:30 a.m., LPA observed the kitchen walls and floor with food particles. LPA Garcia inspected the inventory of food and observed various food products expired (pictures were taken). When LPA Garcia asked Staff #4 when the kitchen was last clean, staff #4 stated 2 months ago.

During the inspection of the classroom sinks and counters all were observed to have dirt build up. The classroom trash bins cover lids were observed with food build up. During inspection of restrooms LPA observed toilet in room #130 not working toilet has no running water.

On March 28, 2023, the facility reported a possible epidemic outbreak (Norovirus) to the department due to six children having vomiting, diarrhea, and abdominal pain. The facility did not report this incident timely, within the required 24 hours. In addition, the facility has not submitted the required written report due on 04/04/2023.

Based on information obtained during interviews conducted with staff and records review, LPA Garcia determined that the facility had an additional 18 cases of gastrointestinal illnesses reported. The facility did not report additional cases to the department.

Based on information obtained during this investigation, the facility failed to implement the written directives provided by the County of Los Angeles Department of Public Health to decrease the spread of gastrointestinal illnesses.

This poses an immediate health and safety risk to children in care. Title 22 Regulation Section 101223 Personal Rights states (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs.

Per facility representative, the facility will be closed from 04/07/2023-04/11/2023 and will re-open on 04/12/2023. In-service training will be conducted with all staff to ensure all staff follow proper procedures to decrease the spread of gastrointestinal illnesses.

The following deficiencies listed on the attached LIC 809 deficiencies page is being cited in accordance with California Code of Regulations Title 22.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). Acknowledgement of Receipt (LIC 9224 form) must be maintained in each child’s file immediately upon receipt from parent.

Lupe Mennillo was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form (English/Spanish/Chinese).

A notice of site visit was given and must remain posted for 30 days.
