32 | Pertaining to the allegation that “Facility is not properly cleaned and sanitized”. Complaint alleges that this matter has been addressed to teachers, and the director, but no action has been taken. Restrooms are disgusting, floors, tables, trash.
LPA interviewed Director who stated; I noticed the chair from that classroom, needs to be cleaned ( LPA observed a urine stain on the children's chair), another statement made by director, I noticed the carpet needs to be cleaned. I noticed sometimes the sinks needs to be cleaned. Its the teachers that use them all the time. LPA observed the bathrooms toilets in room 112, 115. 110 to have yellow ring stains inside and on the outer part of the the toilet, LPA observed the rugs in the classrooms to be dirty, LPA observed the classroom sinks have build up stains. LPA observed the floors have shoe mark stains.
Based on observations, interviews concluded and records review, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, Title 22 101238(a) Buildings and Grounds, is being cited on the attached LIC 9099-D.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
