Licensing Program Analyst, (LPA) Staicy Perry conducted an unannounced Case Management Inspection for an incident that was reported to the department. LPA Perry met with May Sabbah, Site Director. Facility is located inside of the Crescent Villages Apartment complex. Census was taken and their were 24 children present. The purpose of this visit is to follow up on an incident reported to the Department on 9/16/24.

During course of the inspection, LPA Perry was taken on a tour of the facility. All teacher/child ratios were met and criminal record clearance for staff was verified. LPA obtained written notice from the Management office of Crescent Villages apartment complex during inspection.

On 9/16/2024, it was reported to the department via phone call that on 9/11/24 facility was informed and advised by the Management Office of Rising Stars that the NIC Protection Company will be performing a test of the fire and life safety at the property. Fire alarm would be heard at various times during the test procedure and electricity would be shut off. Due to the interruption of the electricity and fire alarm testing, PACE administration chose to cancel all classes for the day. LPA was informed that all families were notified by class Dojo app and verbally during dismissal on Friday 9/12/2024. In addition a notice was posted in the front gate about the school closure.

The incident was reported to the Department within the 24-hour period and the written report was received via email on 9/16/2024. No children were involved. Facility closed due to facility interruptions, no electricity and fire and life safety system testing. LPA was informed that facility reopened the following business day 9/17/2024.

All reporting requirements were met. No deficiencies were cited. Notice of site visit was provided. Appeal rights were discussed and exit interview was conducted with the Site Director.
