An unannounced annual random inspection was conducted on this date by Licensing Program Analyst (LPA) Seung Lee. LPA met with Director Asha Tennakoon. LPA toured the facility. There were a total of 16 children present, along with a total of 5 staff upon arrival.

LPA Lee observed 8 children in the Bumble Bee room with 3 staff members. 8 children in the Lady Bug room with 2 staff members.

The infant component is located on the same site as the preschool component #198018426. The program consists of two classrooms. The infant program has its own separate areas, there are no shared spaces with the preschool program.

LPA inspected the classrooms designated for the infant program. LPA did not observe any hazards or violations in the classrooms. Appropriate cribs and napping equipment were observed in the facility, clean and in good condition. There is a functioning carbon monoxide detector in the facility. Hand-washing sinks are available to staff and children. Potty training is not a service provided in the infant program. Changing tables were observed to be in clean and good condition and within arm's reach of a sink. Younger infants have their food (milk, formula, foods, etc.) brought from home. Toddler children are provided 2 snacks by the facility and bring lunch and an additional snack from home. Filtered drinking water is available to children who use their own personal water bottles. LPA inspected the refrigerators and counter spaces where food is prepared. LPA observed the surfaces to be clean. LPA observed children's bottles and other personal items were labeled.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Play area was inspected for hazards and inaccessibility to bodies of water. LPA did not observe any bodies of water on this inspection. Equipment was observed to be in good condition and clean. Outdoor drinking water is available through use of personal water bottles.

LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. During the inspection LPA Lee inspected and measured 1 classroom that this facility has added for a pending capacity increase application. No additional outdoor space has been added. Fire Clearance has been approved and received by the regional office. The new room is adjacent to the Lady Bug room with its own entrance and exit to the outdoor play space. The additional space granted from the new classroom does meet the capacity requested in the pending application. After LPA Lee observed that no new outdoor space has been added, the Director stated that the facility would like to request a waiver to use a staggered outdoor play space schedule. LPA Lee advised the facility on the required documents for the waiver request during the inspection. LPA Lee advised the Director that a waiver is not always granted, and is approved based on the discretion of the regional office.

There were no deficiencies observed during the inspection.
