An unannounced Annual Required Inspection was conducted on 08/24/2021 at 9:10 AM by Licensing Program Analyst (LPA) Katrina Chicote. LPA conducted COVID-19 screening call prior to entry. Facility is currently licensed for a capacity of 50 children. Licensing staff met with Lizbeth Renova, Director. Upon entry, Director conducted health screening questions and took LPA's temperature, and LPA observed COVID-19 posters posted at entrance. The program currently operates Monday thru Friday from 7:30 AM to 4:00 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. Facility shares space with church on same property. Director states there is a separate entrance for the church and separate hallway, so church goers do not have access to the school.

At the initial start of the site visit, there is a total census of seven children and a total census of four teachers present. The facility is made up of two classrooms and each were observed for cleanliness and safety. Furniture and equipment were inspected for age appropriateness and good repair in both classrooms. LPA observed wooden shelves filled with sensory toys, plastic manipulatives, art materials, books, and other toys free of loose and sharp parts in each classroom. Telephone service, heating, lighting and ventilation were evaluated. LPA observed air conditioning wall units in both classrooms. There were cubbies observed as storage for children's belongings. Mats/cots were inspected, facility uses standard blue cots for napping and LPA observed children's names labeled on each cot. Per Director, linens are provided by the families and taken home weekly. Availability of drinking water was reviewed. At 9:30 AM, LPA observed a stairs in between classrooms that is barricaded, making stairs inaccessible to children in care. Per Director, upstairs is used by the church and preschool does not use upstairs area. LPA observed small water bottles kept on top of cubbies of each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Facility has two bathrooms for use for children. At 9:33 AM, LPA observed Bathroom 2, that Classroom 2 uses, to have 1 sink and 1 toilet and Bathroom 1, that classroom 1 uses, to have 2 sinks and 2 toilets. Bathrooms are located in hallway in between classrooms.

A first aid kit is kept in a cabinet in each classroom. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced in 02/08/2021 in Room 2, fire extinguishers observed in Kitchen and Room 1 were last serviced on 08/19/2020. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. At 9:42 AM, LPA observed Raid Ant killer, germicide, and bleach solutions in a high cabinet in Kitchen, which is inaccessible to children in care. Director states that these items belong to the church and that they share the Kitchen with the church. LPA advised that they label church storage items and suggested a lock be kept on these cabinets during daycare hours.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. At 9:36 AM, LPA observed Kitchen in Classroom 2, to be inaccessible by a closed latch, making area inaccessible to children in care. Lunch is served at 11:30 AM and is brought in from Main Kitchen.

At 9:46 AM, LPA inspected outdoor equipment for safety, cushioning material, good repair and age-appropriateness. Children have access to playground through separate entrances for each classrooms and walk through parking lot to get to back outdoor play area. LPA observed parking lot to be fully fenced and have a gated access entry, per Director anybody entering has to call first prior to gate being opened. LPA confirms as LPA called to be able to gain access to parking lot entry. Required shade and fencing were inspected. At 9:48 AM, LPA observed shaded area where three children were playing on a table with Staff 2. LPA advised Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children bring their own little water bottles for drinking water, LPA observed each child to have a small water bottle at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. CPR card expires in 03/21/2023 for Staff 1. Inspection of required forms made.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Director:
Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited.

The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

LPA informed Director to log onto web site www.ccld.ca.gov to obtain forms and LIVESCAN application. Records for all children and staff must be maintained for three (3) years after separation from the facility.

The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. Director advised to visit www.shotsforschool.org for immunization information.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 08/24/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
