13 | AMENDED REPORT TO CLARIFY TYPO - ALL JULY 20, 2021 DATES SHOULD INDICATE YEAR 2020.
This complaint inspection was conducted by Licensing Program Analyst (LPA) Warren Birks. Due to COVID-19 and precautionary measures, this inspection was conducted via teleconference with Director Twaneisha Baker. LPA observed 6 preschool children during the visit.
During the July 20, 2021 tele-inspection, LPA observed Director Baker’s fingerprint clearance was not associated to the facility (in the Licensing Information System [LIS]). Director Baker informed LPA that she started approximately 1 week prior to July 20, 2021. She also indicated that she called weeks ago to get information to transfer her fingerprints and a Licensing Representative told her she did not have a clearance in the system (which slowed her whole transfer process).
LPA informed Director Baker that Title 22 regulations require all staff be fingerprint cleared and associated before working in a licensed facility. Based on interview and LIS data, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A $500 penalty is assessed. |
