13 | Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced complaint inspection on August 3, 2023. LPA arrived at the facility at 8:10am and met with Elia Rocha, Associate Director, for the purpose of delivering findings for the above allegations. LPA toured the facility with Elia Rocha. LPA observed 5 children in care upon arrival.
LPA Chambers inspected the facility on 05/09/23, collected documents and photos, LPA conducted interviews with staff and parents.Based on interviews and file review, LPA found the center did not maintain clear and thorough documentation of individual services provided for clients exhibiting emotional and behavioral problems to assist in handling crisis or emergency situations as needed. LPA did not observe any written assessment or intervention plans, and no Individual Case Plans or physicians note. LPA learned through interviews, staff #1 and staff #2 utilized a weighted blanket on child #1. Staff denies wrapping child #1 with a weighted blanket. There were no witnesses to corroborate the allegation of restraining or wrapping
