Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced annual inspection on 03/21/2023 at 8:30 am. A risk assessment was conducted prior to entering the facility, appropriate PPE was used. LPA met with Gloria Mejia, Ass. Director, who guided LPA on a tour of the facility. This is an Early Head Start infant program with a toddler option licensed for 96 children. Facility consist of nine (09) classrooms. Infant classrooms consist of Room #5, Room #7, and Room #9. Toddler classrooms consist of Room #1, Room #2, Room #3, Room #12, Room #13, and Room #14. Facility operating hours are Monday to Friday from 7:30AM to 5:30PM. Per Assi. Director, there's 42 children enrolled.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection; Room # 1: Staff #1 with 1 child; Room # 2: Staff # 2 and Staff # 3 with 5 children; Room # 3: Staff # 4 with 3 children; Room # 7: Staff # 5 and Staff # 6 with 7 children; Room # 12: Staff # 8 and Staff # 9 with 3 children; Room # 13: Staff # 10, Staff # 11, and Staff # 12 with 5 children. The facility was observed to be within the license capacity and limitations.

Per Assi. Director classrooms # 3,5 and two bungalows are closed due to low enrollment. Teachers and children from room # 9 were located in room # 3 due to a leak. LPA inspected all classrooms on this date. The following was observed during the tour of the facility:
PHYSICAL PLANT
Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Linens are washed each week or as needed in the facility. Napping equipment (cots) were observed to be stored in a cabinet within the classroom. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in the classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Per Assis. Director, there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no pools or bodies of water at the facility.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC)

FACILITY RECORDS: Children’s Records were reviewed for completeness; Inspection of required forms was made.

LPA partially reviewed staff files during the inspection due to the full staff files being stored at the administrative office. LPA will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

LPA issued the Review of Children's Records (LIC 857) to the facility representative during this inspection. The LIC 857 documents the children’s files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 3/3/2023.



Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Infants and toddlers are provided breakfast, lunch, and a PM snack.

First Aid supplies were observed in the classroom in a cabinet. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the staff office.

This facility provides Incidental Medical Services – IMS. LPA advised the facility representative to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
