An unannounced Case management deficiencies- Inspection was conducted on this date by Susann Sanchez, Licensing Program Analyst (LPA). Upon arrival LPA met with teachers Letica Ramirez- Murry & Delicia Mckie. The purpose of this visit was to address citations observed. Upon LPA's arrival, three infants and six children were observed with two staff in the preschool area. Carina Rivas, Director/ Owner arrived during inspection.

On 10/04/2019 & 12/12/2019, LPA observed Infant Center co-mingling with the Preschool. On both inspections, all the children were gathered in the preschool room. During interviews conducted with staff and parents, discourses were made that children all gather in the morning time (6:00am) in the preschool area. This issue was address on 10/04/2019, per director staff will be coming in earlier at 7:30am so facility is not co-mingling. As preschoolers are supposed to be kept physically separated from the infants and toddlers, the Center is being cited for co-mingling.

LPA also observed a child strapped in a car seat and staff #2 moving the car seat up and down with her/his foot. LPA asked staff #2 to please remove child from the car seat.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Carina Rivas, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

Personal Rights: Children should not to be placed in any restraining device. Postural supports may be used as specified in Section 101223.1. The requirement is not met as evidenced by LPA observed infant strapped into a car seat. This poses an immediate risk to the health and safety of children in care.
