This is an Amended Report
An unannounced Complaint Investigation was conducted on this day by Licensing Program Analyst (LPA) A. Lucero regarding the allegations mentioned above. LPA met with Teacher Consuelo Rosales as Director was unavailable and Head Teacher Veronica Camacho was unavailable at the initial start of the inspection. Head Teacher Veronica Camacho arrived at 12:33pm to conclude complaint investigation with LPA.
Complaint alleges staff mishandled day-care children, staff yelled at day-care children and that the facility is dirty. LPA interviewed currently enrolled children in care, no disclosures were made. LPA interviewed Director and staff who deny the allegations. Interviews were conducted parents of currently enrolled children, no disclosures were made. Regarding the allegation where the facility is dirty, LPA conducted unannounced inspection to the facility on 06/07/2019 and again on 6/14/2019. On both tours of the facility, LPA toured the indoors of the classrooms, the outside play yard, children’s restrooms, and facility kitchen. On both tours, LPA did not observe the facility to be dirty or in need of cleaning. LPA obtained photos of facility on 06/07/2019 and cleaning log.

Regarding the allegations of staff mishandled day care children and staff yelled at day-care children, interviews were conducted with currently enrolled children, no disclosures were made. Interviews were conducted with parents of currently enrolled children, no disclosures were made. Staff deny allegations.

This agency has investigated the complaint alleging staff mishandled day-care children, staff yelled at day-care children and that the facility is dirty. Although the above allegation(s) may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation(s) is Unsubstantiated.

The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview conducted with the Licensee, during which appeal rights were given and explained. A copy of the Appeal Rights (LIC 9058 01/16) was provided. The Licensee’s signature on this report acknowledges receipt of rights.

Due to technical issues, original signature is on file. Director was provided with a hand-written LIC 9099 that has original signatures. A complete copy of this report was mailed out to Director.
