25 | REQUIRED-1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 06/12/23. LPA arrived at the facility at 12:00 PM and met with Marine Vardanyan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, were Karine Vardanyan, Licensee’s Assistant and Vaagn Simonian, licensee's husband. Per licensee, facility hours are Monday to Friday from 7:00 a.m. to 10:30 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home located on the first level. The home consists of 2 bedrooms, 2 restrooms, living room and dining room (main day care area), storage inside the home adjacent to the living room, kitchen, garage, front yard and backyard (fenced). The children will use the bathroom in the hallway, living room and dining room (main daycare area), kitchen, and front area of backyard (fenced). Areas off limits to children and parents include: 2 bedrooms, 1 bathroom, storage inside the home, garage, front yard and back of backyard. During this inspection LPA did not observe any stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee, parents enter the home through the main entrance which leads to the living room or side gate which leads to the backyard.
There is an above ground pool located in the off limit area in the backyard. LPA observed at 12:30 p.m. the pool was covered with wood cover which was strong enough to completely support the weight of an adult. Also the solid bronze decorative two heavy lions statues were observed on the top of the pool wood cover.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:10 p.m., Licensee took this LPA on a tour of the home. LPA was toured REPORT CONTINUES ON NEXT PAGE 1 of 4 |
