An unannounced Required – 1 Year Inspection was conducted on 11/10/2021 at 9:30 AM by Licensing Program Analyst (LPA) Katrina Chicote. At 9:20 AM, LPA conducted COVID-19 screening call prior to entering facility, LPA was able to leave a voicemail and conducted COVID-19 screening call prior to entering afcility. Facility is currently licensed for a capacity of 22. Licensing staff met with Director, Diane Connell. The program currently operates Monday thru Thursday from 9:00 AM to 2:00 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. Entrance to facility is located to the side of building on Grand Ave., upon entering facility you ring doorbell and you are let in by staff upon clearing a temperature check. LPA observed an authorized representative dropping off child upon arrival and COVID-19 health protocols being implemented.

At the initial start of the site visit, there is a total census of eight children and a total census of three teachers present. The facility is made up of one classroom, called the JDE room. Facility is located in bottom floor of apartment complex with multiple rooms available that remain locked during operating hours. At 9:31 AM, LPA observed children's belongings hanging in cubby area in hallway prior to entering the classroom. At 9:33 AM, LPA observed furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. LPA observed child size tables and chairs, carpet and vinyl flooring through out room. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per Director, linens are provided by the parents and taken home weekly. Availability of drinking water was reviewed. Per Director, children bring their own water bottles from home and are refilled as needed by staff. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. At 9:36 AM, LPA observed diaper changing table, two toilets and seven sinks in classroom. At 9:38 AM, LPA observed wooden dollhouse, play kitchen, book shelf filled with books, art supplies, and other child appropriate toys free of lose and sharp parts available for children at time of inspection.

A first aid kit is kept in cupboard above children's nap cubbies. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced in 03/26/2021. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. Per Director, these items are stored in a high cabinet in classroom.

Per Director, facility provides AM snack, which is brought in by staff and children bring lunches from home. LPA observed lunch boxes with children's name labeled hanging in hallway cubby area.

Per Director, facility uses upstairs courtyard area of apartment complex for outdoor play. LPA observed waiver for outdoor play use in facility file during file review. At 9:43 AM, LPA walked up stairs that has a lower handle bar for children to use for walking upstairs. Per Director, staff supervise children as they walk up and down the stairs at all times. At 9:45 AM, LPA inspected outdoor equipment for safety, cushioning material, good repair and age-appropriateness. LPA observed a soft cushioning material through out, child size tables and chairs, and an outdoor rug. Per Director this area is where children eat their snack and lunch. LPA observed plastic playhouse available for children. Per Director, they also store bicycles and other outdoor play toys in storage area, LPA observed storage area to contain bicycles, plastic ride along toys, and other appropriate toys free of lose and sharp parts available for children in care at time of inspection. At 9:47 AM, LPA observed a bathroom available for children in outdoor play area to have an operable child appropriate toilet and sink. Required shade and fencing were inspected. LPA Chicote advised Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

At 10:13 AM, staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. CPR card expires on 07/2023 for Staff 2 (S2). Inspection of required forms made.

The following were discussed with the Director:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 11/10/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
