Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced annual inspection on 09/03/2021 at 9:45AM. LPA met with Rhoshanda Ventura,Teacher, who guided analyst on a tour of the facility. This is an infant program which consists of 1 classroom; Room 4. Facility operation hours are Monday to Friday from 7:45AM to 2:45PM. LPA was later met by Natasha Jackson, Early Learning Center Manager, and Keokuk Largarde, Center Manager. This facility is located within the premises of Beach High School in the city of Long Beach. This facility also has a preschool component with license #198002412.

The infant program is located adjacent to the center's pre-school program. The infant program has no shared areas. All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room 4: Staff #1 and #2 with 4 infants. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. The facility has sufficient napping equipment, feeding chairs, and changing tables. Infants in this program are from the age of 12 months to 24 months. Infants sleep on cots. No infants were observed to be swaddled. Telephone service, heating, lighting and ventilation were evaluated. Infants have their own cubby to store their belongings. Linens are washed weekly by a laundry service. Per Teacher, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in the classroom.


Disinfectants, cleaning solutions, medication and other items that are dangerous to infants, were inaccessible to infants. Teacher states that there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. ------Page 1 of 3

All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. No infants are on formula only solid foods. Breakfast, lunch and snack menu was observed to be posted. Food in refrigerator was labeled. Kitchen counter tops were clean and free of clutter or standing food.

The outdoor playground was observed to be physically separate from the preschool component. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. The outdoor surface is cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to infants were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. Infant Needs and Service plans were reviewed and observed to be in each file. Infants at this facility are above 12 months of age, however, LPA advised the teacher and Early Learning Center Manager that if they decide to enroll children under 12 months they will need to include the LIC 9227 Individual Infant Sleeping Plan on file. When requesting the sleeping log, the facility did not have a log in which infants are physically checked on by staff every 15 minutes. LPA provided and discussed PIN 20-24 CCP and discussed creating a log in which the staff can document whether there is labored breathing, signs of distress, and sleep position (on their backs) for each napping infant. Inspection of required forms was made and documented on the LIC 857.

LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. ----Page 2 of 3

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. Infants are provided with a breakfast, lunch, and snack.

First Aid supplies were observed in the classroom in a cabinet. According to the Teacher, there are currently no infants on medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA advised the Teacher to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA provided the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD. LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.


The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Teacher, Roshanda Ventura, Teacher, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

This requirement is not met as evidenced by the facility not having a 15 minute sleeping log that checks for labored breathing, signs of distress, flushed skin color, increase body temperature, & restlessness. This poses a potential health and safety risk to children in care.
