ANNUAL REQUIRED -1 YEAR INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced random required- 1 year inspection. LPA met with licensee, Kristina Voskanyan at 8:30 a.m. who guided analyst on a tour of the facility. Also present were Ruslan Avetian licensee's father. There were 10 children present during this inspection. Family members residing in the home are 3 adults who have clearances on file and 2 children. Per licensee, adults residing in this address are licensee, and licensee's parents. Per licensee at 9:02 a.m. her parents are residing at this address permanently and they are looking for a place to move out. Facility operation hours are Monday to Friday from 7:30 am to 5:30 pm.

This is a one story home which consists of 3 bedrooms, 1 & 1/2 restrooms, living room (FIREPLACE: which is inaccessible), dining room, kitchen, garage, front yard, backyard (fenced), and basement. The children use 1 bathroom adjacent to the kitchen, kitchen, dining room, living room, one bedroom adjacent to the kitchen, and backyard (fenced). Per licensee, areas off limits to children and parents include: 2 Bedrooms, 1 bathroom, garage, front yard, and basement. The licensee provides food for children in care. Parents enter the facility through the main entrance.

At 9:45 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room where observed a screen was installed to the fireplace to prevent access. During this inspection 10 children were present in the home and playing in the living room. LPA observed electric outlets were covered in the living room. Next LPA was toured through the dining room and observed toys and children table and chairs. Next LPA toured the kitchen and observed safety latches on the cabinets where licensee stores cleaning compounds and sharp items. Next LPA toured the laundry room and did not observe any hazards. Next LPA was toured the 1/2 bathroom adjacent to the kitchen and did not observe any hazards.

Next LPA was toured the 1 childcare bedroom and did not observe any hazards. Next LPA was toured the one off-limit bathroom and 2 off-limit bedrooms and did not observe any hazards. Licensee showed LPA the fire extinguisher was serviced 05/19/2020 as read by LPA from the service tag. LPA observed Fire Extinguisher was anchored to the wall in the kitchen. Licensee also tested the carbon monoxide detector in the hallway of bedrooms and smoke detector in the living room. Both sounded off the alarms and heard by LPA to be functional. During this inspection licensee 's assistant, Nelli Daniekyan arrived to the facility.

Next LPA was toured the backyard (fenced) and did not observe any hazards. Per licensee at 9:15 a.m. children walk through the kitchen to go to the backyard. Children use the fenced backyard area which is covered with artificial grass.

Per licensee at 9:30 a.m. there is a one cat on the premises which is kept in the off limit area. There are no weapons, firearms or bodies of water on the premises. There were toys observed for children. Posting requirements were observed to be posted at the time of inspection in the living room.

Per licensee at 9:35 a.m. there are currently 16 children enrolled, children's roster was reviewed and is current. LPA reviewed five children's files, and reviewed files were completed. Disaster drill log was available, last drill was conducted on 09/10/2020 as it was documented on the disaster drill log.

The licensee was observed to be operating within the licensed capacity and is not exceeding the required limitations during this inspection. All adults present have obtained a criminal record clearance on Licensing Information System (LIS). The licensee has proof of current pediatric first aid and CPR (expires: 10/2022). Licensee completed required mandated reporter training on 09/22/2019.

The following was discussed:


INFANT CARE: Licensee states that she does care for infants. LPA discussed the licensee’s plan for supervising sleeping infants. Licensee states the following: Any infants in care will stay in the area where the licensee or assistant are. LPA advised the licensee to sleep infants where the infant can be directly supervised and advised against sleeping infants in a separate room. The licensee states that she will not sleep infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided.

Medication: Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA explained to licensee that car seat and stroller are only and only for transportation, high chair is only and only for feeding and stated items cannot be misused. Per licensee, she carries two millions liability insurance in accordance with standard established by Family Child Care statue.

LPA advised the licensee at 10:00 a.m. how to access forms, regulations and quarterly updates , and Providers Information Notices (PIN) on line at: www.ccld.ca.gov.

During this inspection LPA discussed PIN 20-06 CCP, Social And Physical Distancing Guidance And Healthy Practices For Child Care Facilities In Response To The Global Coronavirus (COVID-19) Pandemic Written In Collaboration With The California Department Of Education and reviewed Child Care Covid-19 Self Assessment. LPA inquired into the licensee’s practices related to COVID-19. Licensee stated that she practices hygiene such as hand washing with the children. Licensee has hand sanitizer available in the drop off area and inside her home. Licensee states she practices social distancing with children.

LPA issued the Confidential Names List (LIC 811) to the licensee during this inspection. The Confidential Names List documents the children’s files that were reviewed during this inspection.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
Exit interview was conducted with Licensee, Kristina Voskanyan at 10:10 a.m.. Appeal rights explained & provided. A copy of this report and all other Licensing reports must be made available to the public for 3 years.
