Licensing Program Analysts (LPAs) Nolan Tcheng and Suzana Safaryan conducted an unannounced 3-Yr random inspection to the above facility. At 10:50am, LPAs met with Lead Teacher Kelly Callaghan. Also present during this inspection was Licensee's additional Assistant. Per Lead Teacher, the licensees would be back in the afternoon, due to appointment. The teacher states that they currently has 14 children enrolled. A current children’s roster is available and is current. Licensee’s hours of operation are 8am-5pm, Monday-Friday.

This is a two-story home which consists of 4 bedrooms, 2 bathrooms, kitchen, dining room, living room, rear room (Art Room), backyard (fenced), and Guest house (classroom) in backyard. The children use living room (nap room) and rear room (Art Room) in the main house, the backyard, and guest house (classroom). Per licensee, areas off limits to children and parents include: the entire second floor and kitchen. All areas identified on the facility sketch were inspected. The licensee provides food for children in care.

The licensee states that 2 adults and 3 children currently live in the home. Persons residing in the home are identified on the attached LIC811. Teacher states that the Licensee currently have two assistants All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Per licensee, there are no weapons or firearms.

At 11:05am, LPAs were led on a tour of the facility by the licensee. Upon entry through the side driveway gate, the back yard was reviewed. The backyard is large and has ample space for children to play. There are grass areas, small play structure, climbing apparatus, bikes, and plenty of toys and equipment for children in care to use. There are benches underneath a covered patio section that children use for meal time and for activities. The next area reviewed was the Guest house, which is being used as a classroom. There are age appropriate toys and equipment present for children to utilize. At 11:15am, fire extinguisher was observed. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 08/29/2023 as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. REPORT CONTINUES PAGE 1 of 3

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are observed to be posted in the classroom. There is a bathroom available inside the guest house that the children in care use. Bathroom was observed to have two toilets for children of different sizes. Detergents, cleaning compounds, medications, and other items which could pose a danger are inaccessible to children. Smoke and carbon monoxide detectors were tested and are operable. The licensee states that there is a land line on the premises. There are first aid supplies available.

The tour continued to the main house, starting with the Art Room. It was observed to be free of hazards. LPAs reviewed the living room that is used as a nap room. LPAs observed cots for napping use. Where children are less than five years old are in care, stairs are fenced or barricaded. There is a child safety gate which keeps stairs inaccessible to children. Fireplace was observed to be inaccessible to prevent access by children. At this time, families drop off at the back gate of the backyard off Franklin St.

There is another fenced section of the backyard that has a swimming pool. The pool is located adjacent to the house and accessed through the back yard. LPAs entered the area to review the pool. The pool has a 5ft mesh gate separating the pool from back yard. There is a self latching gate that opens away from the pool. The gate's latch is no more than 6 inches from the top of the gate and has a lock on it. It is constructed as to not obscure the pool from view and is strong enough to withstand force against it. There are railings leading down to the fenced pool. Each vertical railing of the gate is no more than 4in apart and the bottom of the gate is no more than 4in from the ground. Teacher states that pool is used only during summer and when Licensee (certified swim instructor) Kevin Neumeier is present. The licensee is observed to be operating within the license capacity limitations.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 11/2025. Licensee and personnel have complete mandated reporter training that expires 1/2024. The licensee does have proof of immunization against influenza, tuberculosis, pertussis, and measles, but facility representative was unable to provide us with the records, due to not knowing their location. Children’s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information specified by regulation.

-There is one large dog that is present at the home. It is accessible to children in care.

-Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family child care facility.

Facility representative was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated

Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Lead Teacher confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Lead Teacher Kelly Callaghan, at 12:55pm. Copy of report provided.
