On August 10, 2023, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced required 1-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Director, Anntoinette Jordan and took a census of the children. There were 9 children and 3 staff members in care. Director had to leave for an hour, and LPA was guided on a tour of the facility by their Lead Teacher. This is a preschool age program licensed for 60 children which operates Monday through Friday from 6:00 AM to 6:00 PM. Per the Director there are 33 children enrolled.

The following classrooms were inspected, Classroom 1:9 children with 3 teachers; Classroom 2: is used as a nap room. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license.

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free.

The facility provides all meals and filtered water to children in care. LPA observed menus posted at the front of the facility and kitchen area. LPA inspected kitchen which is off limits to children in care.

LPA observed that there are functioning smoke and carbon detectors in the facility. LPA observed 2 fully charged and serviced fire extinguisher and was last serve on 10/12/2022. Per Director Emergency disaster drill was last conducted on June 2023 but did not have documentation.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is not being maintained in a safe condition or free of hazards. LPA found trash and sand scattered throughout the play area. LPA did observe a sand box area, which had a long board surrounding the sand area with nails sticking out and a metal piece protruding. LPA took photos. According to the Lead teacher, sand box is going to be removed. Lead teacher stated that staff do come out daily prior to children playing to check for anything hazardous. All areas around or under high climbing equipment, swings, slides, and similar equipment are placed on the cement that does not cushion or prevent an injury to absorb a fall and requires to be moved. There is adequate shade in the play yard. Children are only brought out in the mornings currently due to hot weather conditions.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children’s Records were reviewed to ensure that Identification and Emergency forms and a medical assessment are on file.

Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.

Childcare Center did not complete lead testing prior to their deadline per the Owner of the facility. LPA advised Director that testing needs to be completed. LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information.

LPA reviewed staff records; staff files were not complete. LPA observed a current facility roster. SB792 (Immunization Requirements for Staff and Employees) was discussed with the Director. During the inspection LPA reviewed staff files and found that only 1 out 3 files were found to be complete. 2 Staff members are missing immunizations, TB test, LIC 9052 forms and Mandated Reporting Training Certificates. The Director was found to have all documentation and requirements needed. Director holds a current CPR/1st aid expiring March 2025.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The following deficiencies are being cited in accordance to Title 22 of the California Code of Regulations and Health & Safety codes. Please refer to LIC809D for documentation of deficiencies cited.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and a copy of the report, along with appeal rights was provided to the Director, Anntoinette Jordan.
