25 | Licensing Program Analyst (LPA) Lilli Babcock conducted a Case Management-Deficiencies visit on 11/3/22 to address deficiencies revealed during a complaint investigation. A COVID risk assessment was conducted upon entry and appropriate PPE was used by LPA. LPA met with Director, Jamlia Rivers, to whom the reason for the visit was explained. LPA observed eight (8) children and one (1) staff present at the facility during this inspection.
On 9/1/22, 9/23/22, and 10/20/22 LPA Babcock visited the infant facility. LPA conducted staff interviews with Senior Director and five (5) staff. LPA also obtained several documents related to the complaint allegation, including but not limited to, a copy of the Facility Roster (LIC 9040), Personnel Report (LIC 500), copies of Child Supervision Records (CSRs), staff timecards, and a copy of the Group Me Infant Chat from 8/1/22 to 9/7/22 that revealed that the preschool facility has been commingling preschool children with the infant children.
During interviews conducted, Staff #1 stated that on 8/16/22 there were “6 infants and a handful of preschool students” in the infant room. The CSR for the preschool program for 8/16/22 shows 5 preschool children were signed in, with one staff present between the time of 6:30 am to 7:39 am. The one staff present on the preschool CSR was also the only staff present on the infant CSR showing 6 infants were signed in from 6:30 am to 7:49 am.
Additional staff interviews disclosed the following: Staff #2 stated they had observed an infant being taken to the preschool room for care at the end of the day on more than one occasion. Staff #4 stated
