25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 07/08/2022. LPA arrived at the facility at 8:30 AM and met with Ani Jaloyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Zohrap Hakobyan, Armenui Simonian and Nune Mirabyan, Licensee’s Assistants. Per licensee, Ani Jaloyan facility hours are Monday to Friday from 7:00 a.m. to 10:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee at 8:40 a.m..
This is a one story home which consists of 2 bedrooms, 1 bathroom, kitchen, dining room, living room (FIREPLACE: which is inaccessible), laundry room, and front yard. Main entrance door has two locks and a door alarm. Also there is a screen door with (2) locks on the front entrance. In addition, the wrought iron railings and openings leading to front door are covered with mesh. The children use the bathroom in the hallway, living room and dining room (main child care room), 1 bedroom, and some part of front yard which is separated by trees. Per licensee, areas off limits to children and parents include: 1 bedroom, kitchen, and laundry room. LPA observed two safety gates were installed to the kitchen to make it inaccessible to the children. LPA did not observe any stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 8:45 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms). LPA observed children tables, chairs, and toys in the living room and dining room. There is a fireplace in the living room which has been blocked off to prevent access to children. LPA observed all electrical outlets were covered in the living room and dining room. Next LPA toured the daycare bedroom and observed children napping cots in the bedroom. REPORT CONTINUES ON NEXT PAGE 1 of 4 |
