25 | Licensing Program Analysts (LPAs) Lissete Gonzalez and Roxana Lopez conducted an unannounced Case Management inspection to follow up on an incident that was reported to the Department. Upon arrival, LPAs met with Site Supervisor, Araceli Robles, who provided LPAs a tour of the facility inside and outside. Census was taken.
On 10/30/19, an unusual incident report was made to the department that involved a child in the playground that fell on the sponge where the play structure is located. The child was heard crying and three Teachers walked over to the child and asked the child what happened. The child stated their leg hurt and no mark was observed. The child was unable to get up. Parents were notified and child was taken to the doctor. The child was diagnosed with a fractured tendon. The facility reported this incident to the Department within the required 24 hours.
During this investigation LPAs conducted interviews with staff, inspected the playground, and reviewed video footage of the incident. LPAs also obtained documentation relating to the incident. Based on the information obtained, LPAs determined there was adequate supervision provided the day of the incident and the child was immediately tended to by staff.
Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit. Exit interview conducted with Site Supervisor, Araceli Robles.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
