REQUIRED -1 YEAR and MILITARY INSPECTIONS CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual required and military inspections to the above facility on 08/30/2021. LPA arrived at the facility at 11:05 AM and met with Varduhi Hareyan, Licensee and Syuzanna Jerejyan, Director who guided analyst on tour of facility. This is a preschool program with toddler option which consists of 5 classrooms. Preschool program consists of 3 classrooms, RM #6 (2-3 year old), RM #7 (3-4 year old), RM#8 (4-5 year old), and toddler option consists of 2 classrooms, RM#4 and RM#5 which are empty at this time. This facility is located on Light on the Corner Church property. There is an infant program on the premises with facility number 198019176. Facility operation hours are from Monday to Friday from 6:30 AM to 6:30 PM.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: RM #6: 12 preschoolers with 2 staff, RM #7: 15 preschoolers with 2 staff, RM #8: 9 preschoolers with 1 staff. The following was observed during tour of facility:

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that there are no poisons on the premises. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing sinks are safe, sanitary and are operating properly. All floors are clean and safe. There are two separated bathrooms for toddler and preschool programs.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. There are two kitchens on the premises. One kitchen is located on the first floor and one on second floor. Per director they only use the kitchen in the first floor, there is a stove, refrigerator, microwave in kitchen area on the first floor. The sink in the first floor has hot and cold running water. Cabinets are available for storage.

LPA observed a safety gate was installed to the stairs which leads to the second floor to make the second floor inaccessible to the children. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. LPA observed water pitchers, water dispensers and disposable cups in preschool classrooms. Per director during outdoor play time teachers take water pitchers and disposable cups for children to drink freely.

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Parents take linens home on Friday and returned on Monday. The Director's office will be used by ill children. There is a napping cot available for an ill child. The staff restroom will used by ill children.

Teacher child ratios were observed and staff names recorded. During this inspection there were 36 children present in the facility with 5 staff. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. During this inspection all present 36 children were singed in. All children were observed to be under visual supervision of a teacher at all times.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, swings and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There are astro turf, sand box and rubber chip play areas in the preschool outdoor play ground. Per director, currently children do not use rubber chip and sand box in preschool outdoor playarea. Per director, currently preschoolers only utilize astro turf area for outdoor play area. There are astro turf and rubber areas in the toddlers outdoor play area. The outdoor activity area is surrounded by an iron fence to separate the preschool, toddler and infant programs.

First Aid supplies were inventoried. A first aid kit is kept in the office. Carbon monoxide detectors and smoke detectors are present and in operable condition. Fire extinguishers have been serviced in 07/23/2021.

The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this inspection. Director states there are no pets, weapons or firearms on the premises.

Staff and Children’s Records were reviewed for completeness; Inspection of required forms was made.

The licensee’s email address was obtained during this inspection. The licensee was advised that email is public information.

LPA advised the licensee and director to access forms, child care videos, regulations, quarterly updates, and providers information notices (PINS) on the Child Care Licensing website at: www.ccld.ca.gov. A copy of Effects of Lead Exposure flyer was provided and explained to the director during this inspection.

LPA inquired into the director's practices related to COVID-19. Director stated that she practice hygiene such as hand washing with the children. Director has hand sanitizer available in the drop off area and inside her home. Director states they practice social distancing with children, take children temperature daily. Director, staff, children, and parents will wear masks all the times indoors.



The following was discussed:
· Reporting Requirements.
· Individuals who are 18 years of age or older living in the home must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately. Failure to obtain a criminal record background check clearances prior to initial presence in the home will result in an immediate $100.00 dollar or more per day Civil Penalty.
· The facility shall ensure that each child is accorded the following personal rights: to be accorded safe, healthful and comfortable accommodations.
· All personnel shall be in good health and shall be physically and mentally capable of performing assigned tasks; personnel that pose a threat to the health and safety of children shall be relieved of their duties.
· All personnel shall be given on the job training in housekeeping, sanitation principles and universal health precautions.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Varduhi Hareyan, Licensee and Syuzanna Jerejyan, Director at 2:45 p.m..

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence is on file. Educational background, training, and/or experience, and health screening form for each staff present are on file and were reviewed.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. During this inspection LPA reviewed 15 present children files from 36 total present children, and all 15 reviewed files were reviewed.

LPA observed proof of immunization against influenza, pertussis, and measles in reviewed five staff files.

Mandated reporter training certificates were available on five staff reviewed files.

Children's roster was reviewed and is current. Disaster drill log was available, last drill was conducted on 08/02/2021.

Snack menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides AM and PM snack, and lunch in individual style. Per director, she prepares lunch everyday fresh at the facility.

Medication: There are no children enrolled that require IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
