Licensing Program Analysts (LPAs) Crystal Green and Mary Silva conducted an unannounced annual inspection. LPAs met with Director, Syuzanna Jerejyan and Licensee, Varduhi Hareyan who provided analyst a tour of the facility.
This is a preschool program with toddler option which consists of 5 classrooms. Preschool program consists of 3 classrooms, RM #6 (2–3-year old), RM #7 (3-4 year old), RM#8 (4-5 year old), and toddler option consists of 2 classrooms, RM#4 and RM#5. This facility is located on Light on the Corner Church property. There is an infant program on the premises with facility number 198019176. Facility operation hours are from Monday to Friday from 6:30 AM to 6:30 PM.

The following staff was present during this inspection: RM #6: 15 preschoolers with 2 staff, RM #7: 12 preschoolers with 1 staff, RM #8: 12 preschoolers with 1 staff. LPA observed 10 toddlers with 2 staff outside in their designated playarea. Teacher-child ratios were observed. All children were observed by licensing staff to be under visual supervision of a teacher at all times.

The following were observed during this inspection: Licensing staff observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. There are two separated bathrooms for toddler and preschool programs. There is drinking water accessible to the children in care via personal sippy cups. At this time, the Director’s office is used as an isolation area. Parents are contacted immediately when children are determined to be ill, and staff are ensuring that children with obvious symptoms of illness are not being accepted.

The facility provides AM and PM snacks and lunch. There are two kitchens on the premises. One kitchen is located on the first floor and one on second floor. Per director they only use the kitchen on the first floor, there is a stove, refrigerator, microwave in kitchen area on the first floor. Cabinets are available for storage.
All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment, swings and slides have cushioning material to absorb a fall.

Staff Records were reviewed to ensure that a health screening report is on file. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. During review of staff records, Staff #3 and Staff #4 files did not contain current proof of verification for CPR and Mandated Reporter Training. During review of children’s records, 1 of 8 files reviewed were missing Identification and Emergency form and Physician Report.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiencies that are being cited and need to be cleared to protect the children’s health & safety.
