Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual/required inspection. LPA met with director, Syuzanna Jerjyan who guided analyst on a tour of the facility. The purpose of this inspection is to ensure that licensee is in compliance. On 01/24/18, an office meeting was held with the licensee. At this time, the licensee agreed to ensure the following:

· Licensee will ensure that the required staffing ratio and capacity is met at all times. Licensee was observed to be in compliance with staffing and ratio requirements during this inspection.

This is an infant program which consists of two classrooms; RM #1(0-12 months) and RM #2 (12-24 months). This facility is located on Light on the Corner church property. Facility operation hours are Monday to Friday from 6:30 AM to 6:30 PM. There is a preschool program with Toddler option on the premises with facility number is 198019175.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: RM #1: 9 infants with 4 staff, ; RM #2: 10 infants with 3 staff. The following was observed during the tour of facility:

PHYSICAL PLANT: Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that there are no poisons on the premises. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing facilities are safe, sanitary and are operating properly. All floors are clean and safe. At this time, the office is used as an isolation area.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. Kitchen area, bottle warmers and refrigerators observed to be clean. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed to be free of flies, other insects and rodents. Each infant has a storage bin with his/her name. Hooks are also available to hang coats and sweaters in each classroom.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Infant outdoor playarea is physically separated from the other components at this facility. No pets, fire arms or weapons, or bodies of water on the premises.

INFANT PROGRAM (in addition to general compliance)
Infant changing tables have a padded surface no less than one inch thick and are covered with washable vinyl or plastic and have raised sides at least three inches high. Toys were observed to be clean and safe. Infant napping equipment meets the requirements of 101439.1 (a) - (f). Indoor activity space for this infant program is physically separate from the other components at this facility, and meets the requirements of 101438.3 (a) - (e).

The facility is in compliance with the staff-infant ratios required in 101416.5 (a) - (e). Individual Feeding Plans are on file for each infant that meets the requirements of 101427 (b)(1) - (5). Infant Needs and Services Plan are on file for each infant that meets the requirements of 101419.2 (a)-(b) and 101419.3 (a). LPA reviewed 10 random children files and all had Infant Needs and Services plan on file. Teachers present have proof of the required infant units on file and meet the qualification requirements of 101416.2 (a), (b), (c) and (g).

FACILITY RECORDS: All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection (expires: 09/2020). The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence is on file. Educational background, Health Screening , mandated reporter training certificate, training, and/or experience for each staff present are on file and were reviewed. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.
Children's roster was reviewed and is current. Infant and preschool have their own separated rosters. Sign in and out sheets were reviewed. All children present were observed to be signed in. Infant program has its own sign in and out book by infant classroom room. Disaster drill log was available, last drill was conducted on 01/06/2020.

Infants 0-12 months and 12-24 months bring their own food. The facility shall ensure that each infant is never left unattended, and is under the direct visual supervision of a staff person at all times.

Medication: There are no children enrolled that require IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Staff and Children’s Records were reviewed for completeness; Inspection of required forms was made.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided.


There were no deficiencies cited during today’s inspection.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Syuzanna Jerejyan, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. A copy of this report and all other Licensing reports must be made available to the public for 3 years.
