Licensing Program Analyst (LPA) Cynthia Reyes, conducted an announced Inspection to measure the requested Toddler Option for capacity determination. LPA met with Pearl Omiya, Licensee. Due to COVID- 19 precautionary measures were taken during the entire inspection and all individuals present during this inspection wore appropriate personal protective equipment. The licensee is requesting a capacity of 15 toddlers (18 to 36 months) added to the already licensed infant capacity of 8. Days & Hours are Monday-Friday 630 AM-630 PM.

At 11:15 AM, LPA began the tour of the facility with the licensee Pearl, beginning with the entrance to the entire facility from the back of the facility parking lot where parents will enter through the large gate and walk pass the out door play yard to arrive at a table located at the bottom stairs that lead to the second story of the facility where the Toddler class rooms are located. The table will be used at this time for Covid for parents to sign the children in/out and temperatures will be taking of both parents and children. Staff will then lead the children up the stairs to enter the class rooms. Parents will not enter the class rooms for toddlers or infants at this time. (Infant class rooms are located on the first floor of the facility at the bottom of the stairs of the toddler class rooms.)

LPA observed the bottom of the stairs leading to the toddler class rooms, has a small wrought iron gate with a self latch on top to keep closed. The licensee modified the original adult stairs to smaller child steps and added extra child size railing to hold on to while climbing up and down the stairs. The toddlers and adult will need to go up and down two flights of stairs with a small portion for stopping if needed which is located in between each flight of stairs. LPA and licensee proceeded to enter class room 1 at the top of the stairs and when entering the class room a small portion is closed off for parent to sign in/out after Covid and children's cubbies are located in this same area. The parent board is also located in this area with all the required posting observed. Class room 2 entrance is identical to class room 1.

1130 AM During the tour of the class rooms, Toddlers, Furniture and equipment were inspected for age appropriateness and good repair. LPA observed napping mats are located in a storage cabinet in the kitchen for class room 1 and for class room 2 they are located in a cabinet in their own class room. There is accessible drinking water readily available, by use of a gallon water bottle with pump. Toddlers will bring their own Sippy cups labeled with their name.

LPA observed in class room 1, a large sink for adults and 2 small sinks for the toddlers. Behind the toddler active space in room 1, is a small kitchen with a gate to prevent children access to that area unless walking through to the bath room escorted by a staff member. LPA observed in room 2, a large sink for the adults and 2 small sinks for the toddlers. In class room 2 behind the activity space is a small bathroom with 2 toilets and 1 sink and a changing table that is located across from the sink, however barley is in arms reach. (LPA to discuss with supervisor prior to licensing.)

In both class rooms toys were observed to be clean and safe and are age appropriate. Telephone service, heating, lighting and ventilation were evaluated. Toddlers each have their own cubby to store their belongings. Children own linens will be washed by the facility every night. LPA observed the facility has a carbon monoxide detector and smoke detector in each classroom. There is a serviced fire extinguisher (06/18/2020) available in each class room. Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible at all times and located in locked cabinets in each class room

This program will provide AM & PM snack and the parents will bring the child's own breakfast and lunch.
A monthly snack menu is available and posted. LPA advised the licensee to label and date all food, dishes and bottles that are brought from home for proper storage. No transportation is being conducted by the facility.

All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. First Aid kits are available in each classroom and were observed to contain the required first-aid supplies.

An ill/isolation area for toddlers is located in the directors main office with a pack n play for resting. Ill children will use the adult restroom which is located right outside the main office.

Outdoor play equipment was inspected for safety, free of sharp, loose pointed parts, good repair and age appropriateness. The surface of the outdoor activity area was observed to be maintained in a safe condition, and free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. Required shade and fencing were inspected. Drinking water will be available by children useing their own Sippy cups. The playground was inspected to ensure no bodies of water or hazards which were not observed. (Waiver is being request to share the Infant play yard)

Items discussed during this Inspection:
Current Children’s Roster LIC 9040: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This children roster must be updated and available to the licensing agency upon request.
Pediatric First Aid and CPR: At least one person in the facility, along with the Director/Administrator must be trained in Pediatric First Aid and CPR and present in the facility.
Qualifications and staff files: Educational background, training, and/or experience and all required documents by the department for each staff present must be available for review.
Children’s Records: All required licensing documents must be available for review; including but not limited to, the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Director advised to visit www.shotsforschool.org for immunization information.
Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or
volunteer at a day care center if he or she has not been immunized against influenza, pertussis, & measles.
Criminal background clearance: LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.
Reporting Requirements Section 101212: Changes at the facility must be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

A fire clearance was approved and on file for a capacity of 15 toddlers and 8 Infants on 01/04/2021.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. Infant Care: LPA advised the applicant to sleep infants where they can be directly supervised at all time. LPA advised against sleeping infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided. LPA discussed and provided a copy of PIN 20-24-CCP:

1:50 PM LPA advised the applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA also consulted with applicant regarding COVID-19 health and safety guidelines on this date. Applicant was provided forms to post via email. Licensing staff observed COVID- 19 postings posted in different areas of the day care on this date. During this inspection LPA also discussed PIN 20-06 CCP, Social and Physical Distancing Guidance and Healthy Practices for Child Care Facilities in Response to The Global Corona virus (COVID-19) Pandemic Written in Collaboration with The California Department of Education. LPA reviewed with the applicant the Child Care Covid-19 Self-Assessment guide the applicant filled out and sent into the department on 12/15/2020 before this inspection was conducted.

Based on the LPA observation, the following corrections need to be corrected before a license can be given.


1) Waiver approval for toddlers to share play ground with the infants. 2) Changing table with in arms reach of a sink. 3) Final file review 4) Second story approval. (Photos taken)

Based on LPA measurements taken on this date, the capacity for 15 toddlers inside is approved and 9 toddlers outside. Toddlers do not have their own yard and a waiver is requested.

Exit interview was conducted in person with Pearl Omiya, Licensee. A copy of this report and Appeal Rights were given and discussed, signed and printed on this date. Consultation regarding the licensing state laws and regulations, were conducted through out this inspection.
