An unannounced Required – 1 Year Inspection was conducted on 09/12/2022 at 9:15 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 48. Licensing staff met with Mayra Rubio, Site Supervisor. The program currently operates Monday thru Friday from 6:30 AM to 5:30 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors.

At the initial start of the site visit, there is a total census of 36 children and a total census of six teachers present. The facility is made up of two classrooms, Classroom 1 and Classroom 2. LPA observed Classroom 1 to have Staff #1, #2, #3 with twelve preschoolers and Classroom 2 to have Staff #4, #5, and #6 with 24 preschoolers.

Beginning at 9:20 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. Required postings were observed in office as well as each classroom. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats were observed stored in each classroom. Per Site Supervisor, sheets are provided by the facility and washed weekly and blankets are brought from home and taken home weekly. Availability of drinking water was reviewed. LPA observed water filtration system available for each classroom. LPA observed learning stations with toys for dramatic play, math manipulatives, art area, and reading area for quie tplay available for children in care that are in good repair free of loose and sharp parts. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Classroom 1 has one toilet and one sink in classroom available for children and other Bathroom, located in hallway outside classroom, to have three toilets and three sinks available for children.

LPA observed a first aid kit is kept in a backpack hanging in each classroom. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced in 04/08/2022. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed disaster drill log available and last drill conducted on 08/31/2022.

At 9:50 AM, Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed Classroom 1 to be using outdoor play during inspection with three staff and 12 preschoolers. LPA observed large play structure with slide well maintained with cushioning material below to absorb fall, bicycles with concrete bike path, and art materials available for children in outdoor play area. Required shade and fencing were inspected. LPA Chicote advised Site Supervisor that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed the children have access to water cooler for drinking water while outside. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 9:57 AM, food preparation areas were toured for safety, cleanliness and proper equipment. Facility serves Breakfast, Lunch, and AM/PM Snack.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed 36 of 36 children signed in with full signatures of parent. Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. CPR card expires in 01/29/2024 for Staff #6. Inspection of required forms made.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Site Supervisor:
Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 09/12/2022.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
