On 12/10/19 at 11:55am, LPA Adrian Risher, Lourdes Castellanos and Ericka Hill conducted an unannounced complaint inspection to deliver the findings for the above allegations. Upon arrival, LPA met with the Angela Buck, Director. LPA explained the purpose of the inspection. LPA toured the facility with the Director and observed 10 children in care with 3 staff present in room 1 and 10 children in care with 3 staff present in room 2.
LPA previously conducted interviews with Director 1, Director 2, and Staff 1 to 8. LPA conducted two unannounced visits on 11/20/19 and 12/04/19. LPAs observed room 1 and room 2 within ratio during each visit. LPA observed 11 infants with 3 staff in room 1 and 8 infants with 3 staff in room 2. During the interviews, staff reported that Director 2, Staff 1, Staff 4, Staff 8, and 1 additional staff member are used as back up and cover breaks for staff assigned to room 1 and 2. Director 2 stated that Staff 1 was assigned to room 1 while they are searching for permanent staff. Staff informed LPAs that no staff is left alone with more than 4 children and the teacher’s aides are not left alone.

During the previous visits on 11/20/19 and 12/04/19, Staff informed LPAs about their diapering procedures. Staff stated that the changing table is sprayed and cleaned after each diaper change. Staff stated that they use medical paper to cover the changing table. Staff stated that they gather the supplies for diaper changing and places the child on the changing table. The poopy diapers are placed in a bag before throwing it away. One teacher changes the diapers and one teacher helps the children wash their hands. LPA Adrian Risher observed a stocked changing table in arm’s reach of a sink in Room 1 and Room 2. The changing table was clean and organized.

During the previous visits on 11/20/19 and 12/04/19, Staff stated when children are crying or fussy, they hold, soothe the infants and are very gentle with the infants. Staff also stated that they talk to the children or provide them with teethers when they are fussy. On 12/04/19 at 10:15am, LPAs observed Staff 7 holding and soothing an infant who was fussy and crying in room 2. LPAs also observed staff reading books and playing with age-appropriate toys with the infants in room 1 during the visit on 12/04/19.

Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

Exit interviews was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
