25 | Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management Incident inspection at the above facility on 02/20/25 at 02:25 p.m. The purpose of this inspection is to follow up on an incident reported to the Department on 02/13/25. LPA met with Site Supervisor Graciela Cerezo who guided LPA on a tour of the facility. Census was taken.
During this inspection, LPA interviewed Staff 1 (S1). LPA reviewed child 1 (C1) file and diaper logs dated 02/03/25 – 02/20/25. Copies of diaper logs were obtained.
On 02/13/25 the parent of C1 left a voicemail at the facility stating that they would be taking C1 to the hospital and that the child would not return to the facility. Facility was not aware as to why the parent was removing C1 since staff were not aware of any injuries or incidents. According to the parent, C1 was crying at home and pointed to the diaper. Parent checked C1s diaper and observed dried feces in the child’s private area. At 04:50 pm staff received a message from the director who stated that they spoke with the parent of C1 and confirmed that the child had a diaper rash. According to Staff 1 (S1), diaper changes are conducted every 2 hours or as needed, and diaper logs are documented in real time. Per S1, there were no visual signs of any rash when changing C1s diaper on 02/13/25 or days prior. S1 stated that during pick up, staff review the daily report with parents which includes diaper changes. According to S1 during pick up parent of C1 checked the diaper and said it was ok. On other occasions, S1 stated that during pick up parent of C1 is asked to check the diaper; however, parent says no because they live 2 minutes away. C1 is present at the facility until 12:00pm and sometimes leaves before 12:00pm. S1 also revealed that the facility has a procedure on how to change diapers and it is followed daily. C1 returned to the facility on 02/18/25; however, there was no ointment provided to treat the diaper rash. A procedure for diaper change was given to C1s parent for review. Facility has implemented additional steps for diaper changes to prevent future incidents. LPA reviewed diaper logs dated 02/03/25 – 02/20/25 and observed that diapers are checked or changed every 30 minutes up to 2 hours.
