An unannounced annual site inspection was conducted by Licensing Program Analysts (LPAs) Ariel Cazares and Thelma Razo who met with Site Director/Teacher Susan Tran. This facility is located on the property of Baldwin Elementary School. Access can be gained from the main office located on Almansor St. Hours of operation are Monday, Tuesday, Wednesday, Friday 1:30-6pm and Thursday 12:50-6pm.

Upon arrival, LPAs observed 2 staff with 21 children. LPA found that facility is operating within ratio. The facility consists of 1 portable (#59) and a playground. The areas utilized by the program are exclusive to the facility and not shared during their hours of operation. LPA inspected all areas of the facility used by children.

Furniture and equipment were inspected for age appropriateness and its condition. Telephone service, heating, lighting and ventilation were evaluated. Storage space in form of cubbies and hanging hooks for children's belongings was inspected. Children's restrooms are located outside across from the portable. The condition and age appropriateness of children’s sinks and toilets were inspected. LPA evaluated general sanitation and cleanliness of facility. Availability of indoor drinking water was observed in form of a water jug and disposable cups. LPA and director were not able to verify the presence of a carbon monoxide detector to be tested. LPA advised director to follow up with the principal or education coordinator for having one installed in the facility. First aid supplies and medication are stored in a locked cabinet in the classroom.

Outdoor playground and equipment were inspected for safety, cushioning material, condition and age appropriateness. The playground is properly fenced. LPA did not observe a source of shade at the time of inspection. The tables and benches available for rest are not covered by any source of shade. Play area was inspected for hazards and inaccessibility to bodies of water. Outdoor drinking water is provided in form of water fountains which were functional at time of inspection.

The facility provides the program with an afternoon snack which is provided by the school and eaten in the cafeteria. Children are escorted and supervised by staff when they transition to the cafeteria. Snack menu was observed posted in the facility.

Required postings were observed on a parent board. Director's First Aid/CPR certifications were reviewed and expire 01/17/2020. LPA reviewed sign in/outs sheets and children's roster. The last disaster drill conducted was on 2/7/2020 and are being documented every month.

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s personnel and administrative records. An exit interview was conducted with Site Director Susan Tran. A copy of this report and appeal procedures were explained and provided.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

The outdoor activity space shall be situated to: (1) Provide a shaded rest area for the children.
This requirement has not been met as evidenced by LPAs observation of the playground not having a shaded rest area. This poses a potential risk to the health and safety of children in care.

Carbon monoxide detectors required; inspection

Every licensed child day care center shall have one or more carbon monoxide detectors in the facility that meet the standards established in Chapter 8 (commencing with Section 13260) of Part 2 of Division 12.

The department shall account for the presence of these detectors during inspections.
This requirement has not been met as evidenced by LPA not observing a carbon monoxide detector or being able to verify the presence of one. This poses a potential risk to the health and safety of children in care.
