32 | No disclosures were made by staff as to any infant having a diaper rash due to diaper not being changed regularly. During LPAs inspection the facility was operating within ratio. Based on information received and reviewed for the date in question, it cannot be determined if the facility was operating out of ratio at any given time during the hours of operation. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
The Notice of Site Visit (LIC 9213)-must remain posted for 30 days. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Annie Gekozyan, including but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. |
