On 04/09/2024 at 10:00 am, Licensing Program Analyst (LPA) Crystal Green conducted an unannounced case management other inspection. Licensing staff met with the Licensee, Gabriel Ross. A census was obtained.

The reason for today’s inspection is due to the licensee request to temporarily remove the Blueberry classroom from the Preschool license 198019498 and allow infants from the Lemon classroom ages 18 months to 2 years old to utilize the Blueberry classroom until the end of June 2025. Per Facility Representative, the classroom is currently not being used by any programs.

During this inspection, Licensing staff inspected the Blueberry classroom observed the indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. Drinking water will be available in the classroom via filter drinking water fountain. LPA Green observed the classroom to contain 2 toilets and 2 sinks. The Lemon classroom will temporarily share the Sandbox, bicycle path, and grass area in the outdoor play ground located directly outside of the classroom. No measurements were obtained during this inspection. LPA Green informed facility representative to notify the department if usage of the Blueberry classroom will need to be extended.

An exit interview was conducted, and the report was reviewed with the Facility Representative, Gabriel Ross.
A notice of site visit was given to the facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
