Licensing Program Analysts (LPAs) Rita Ramos and Lilli Babcock conducted an unannounced required 1 year inspection on 09/02/2021 at 1:15 PM. LPAs met with Nancy Perez, Coordinating Teacher, who guided analysts on a tour of the facility. This is a preschool program which consists of 2 classrooms; Rooms 227 and 229. Facility operation hours are Monday through Friday. Both classrooms operate 2 sessions. The AM sessions are from 8AM to to 11AM and the PM sessions are from 12PM to 3PM. This facility is located within Dooley Elementary School in the city of Long Beach.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room 227: Staff #1, #2 and #3 with 3 preschoolers and Room 229 had no children or staff present. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children do not nap at the facility due to part-time enrollment. Per Coordinating Teacher, the isolation area is located in the nurse's office, however, due to low enrollment, Room 229 can also be used as a temporary isolation area due to COVID-19. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. LPAs advised the Coordinating Teacher to post signs on the restrooms to show which restrooms are designated for preschool children as well. Signs were posted during the inspection. Availability of indoor drinking water was observed in classrooms via a water pitcher and disposable cups. The facility does have functioning drinking faucets, however, due to COVID-19 the facility is providing disposable cups and water jugs.



Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. -----Page 1 of 3

Coordinating Teacher states that there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. When inspecting the play yard, LPAs observed that play houses were clean, there is a cushioned area that can absorb a fall. There is adequate shade in the play yard. Per Coordinating Teacher, the facility brings out the water pitcher and disposable cups when outdoors as well even though the water faucets outdoors are also functioning. LPAs advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. The name, address, and telephone number of child’s authorized representative are on file and also a medical assessment.

LPAs reviewed partial information for each staff and their filing records during the inspection. LPAs were able to review that all staff present have proof of the AB 1207 Mandated Reporter certificate on file and staff present have been given on the-job training on sanitation principles, housekeeping, including universal health precautions. LPAs were not able to review immunization records due to those records being stored at the administrative office.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in.

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Preschoolers from the AM session are provided with a full breakfast and children from the PM session are provided with a full lunch.

First Aid supplies were observed in the classroom in a cabinet. According to the Coordinating Teacher, there are currently no children on medication but if a child were to be on medication, medication would only be administered to a child when accompanied with a doctor's note and stored in the child's classroom in locked metal boxes.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPAs advised the Coordinating Teacher to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPAs provided the Head Teacher with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.


There were no deficiencies cited during today’s inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
