NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analysts (LPA) Seung Lee and Anomeh Eivazian conducted an unannounced case management inspection. Upon arrival LPAs met with Director Lena Haiek. The purpose of the inspection was to inspect and measure the new classroom and outside play space for a pending capacity increase application. A full tour of the the entire facility was provided during the inspection. LPAs observed that the facility has a total of 7 sinks and 6 toilets. The new classroom was observed to have 2 sinks and 2 toilets for children. The classroom was observed to have overhead lighting and windows. LPAs observed age appropriate toys and equipment in the classroom. Napping cots were observed to be available in the classroom. The outside play space outside the new classroom connects with the existing outside play space of the facility. The new outside space was observed to have shade. The new outside space was observed to have a fence facing Kenneth Rd and a wall on the adjacent side. During the inspection LPAs measured the indoor and outdoor activity space of the new new areas. Fire Clearance has been received by the department and has been approved for 85 children. Based on the 2 sinks and 2 toilets in the classroom the new space allows for 30 children. The indoor activity space was measured for 781.76 sqft which comes out to 22 children. The outside play space was measured for 1668.50 sqft which comes out to 22 children. Based on all of these factors the new space added allows for 22 additional children in addition to the existing capacity. Report Continues.

During the inspection, The Director was advised that this report along with the application will be submitted for a final review to the department and the facility can only operate under the new capacity after the application is approved.

The following corrections will need to be provided by the facility prior to the application being submitted for a final review.

The two parking stoppers on the ground will need to be removed from the outside play space.
The two parking poles on the ground will need to be removed from the outside play space.

Exit interview conducted with Director Lena Haiek. Appeal rights discussed and explained. The notice of site inspection must remain posted for 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars.
