25 | Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 7/15/24. LPA met with Assistant Director, Elizabeth Ruiz, to whom the reason for the visit was explained. Ms. Ruiz guided LPA on a tour of the facility. Census was taken. There were four staff present caring for 25 children. The facility was observed to be operating within the license capacity limitations.
On July 22, 2024, an unusual incident report was made to the Department regarding an incident that involved a child requiring medical attention. Based on information obtained during interviews conducted with staff, LPA determined that during nap time, Staff observed Child #1 was shaking and unresponsive. 911 was called, the fire department arrived at the facility, and Child #1 was transported to the hospital. Staff #2 stated the mother of Child #1 reported the child had a seizure. Child #1 returned to the facility on 7/23/24. Based on interviews with staff, there was adequate supervision at the time of the incident. It appears that although staff were present, they could not prevent the incident.
Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported. No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
